| Abstract: | Background: One of the most frequent complications of thyroid surgery is hypocalcemia due to postoperative hypoparathyroidism caused by its devascularization or excision. Symptoms of hypocalcemia range from mild paresthesias, cramps to tetany, convulsions, arrhythmias, and even death.
Objective: To determine the frequency of hypocalcemia after total thyroidectomy in patients from the Cancer Institute SOLCA - Cuenca, in the period 2000-2016.
Methodology: A transverse analytical study with information from 423 clinical records of the SOLCA Institute - Cuenca, period 2000 to 2016, collected in a form, tabulated in the SPSS program, the results of which were presented in tables and graphs analyzed qualitatively (f,%) and quantitative (, DS).
Results: 90.1% of the patients were female, mean age in the group was 48.4 years (SD 15.4). 83 patients had postoperative hypocalcemia, giving an overall prevalence of 41.1%, of which 83.1% was transitory, while the overall prevalence of definitive hypocalcemia was 6.9%.
Conclusions: The majority of thyroidectomized patients were adult women; transient hypocalcemia was high but most normalized calcium values after 6 months of surgery. Hypocalcemia was higher in patients undergoing thyroidectomy and group IV lymph node dissection and in patients with malignant lesions. The presence of clinical symptoms showed an association with laboratory-measured hypocalcaemia and, the paresthesias and the sign of Chevostek were the most frequent clinical signs |
| Description: | Antecedentes: Una de las complicaciones más frecuentes de la cirugía tiroidea es la hipocalcemia debido al hipoparatiroidismo posquirúrgico provocado por la devascularización o exéresis de la glándula paratiroides. Los síntomas de hipocalcemia varían desde parestesias leves, calambres hasta tetania, convulsiones, arritmias e inclusive la muerte.
Objetivo: Determinar la frecuencia de hipocalcemia pos tiroidectomía total en pacientes del Instituto del Cáncer SOLCA - Cuenca, en el período 2000-2016.
Metodología: Estudio analítico transversal con información de 423 historias clínicas del Instituto SOLCA - Cuenca, período 2000 al 2016, recolectada en un formulario, tabulada en el programa SPSS, cuyos resultados se presentaron en tablas y gráficos analizados de forma cualitativa (f, %) y cuantitativa ( , DS).
Resultados: El 90,1% de pacientes correspondían al sexo femenino, el promedio de edad en el grupo fue de 48,4 años (DS 15,4). 83 pacientes presentaron hipocalcemia postquirúrgica dando una prevalencia global de 41,1%, de ellos el 83,1% fue transitoria, en tanto que la prevalencia global de hipocalcemia definitiva fue 6,9%.
Conclusiones: La mayoría de pacientes tiroidectomizados fueron mujeres adultas; la hipocalcemia transitoria fue alta pero la mayoría normalizaron valores de calcio tras 6 meses de la cirugía. La hipocalcemia fue mayor en pacientes sometidos a tiroidectomía más disección ganglionar del grupo VI y en pacientes con lesiones malignas.
La presencia de sintomatología clínica mostró asociación con hipocalcemia medida por laboratorio y, las parestesias y el signo de Chevostek fueron los signos clínicos más frecuentes |
