Abstract: | The objective of the present study was get a population pharmacodynamic model of losartan in hypertensive patients treated in the office of the Municipal Autonomous Decentralized Government (GAD) of Gualaceo, by means of mixed effects models, associating them with the covariables of each patient.
Fifty hypertensive patients of both sexes, from 41 to 90 years of age and who met the established inclusion and exclusion criteria, were evaluated.
The values of systolic blood pressure (SBP), diastolic blood pressure (DBP) and pulse were obtened; before and after the administration of losartan in a random manner, within a range of 1, 2, 3 and 6 hours post-dose. In addition, all relevant information was collected, such as his medical history and anthropometric measurements. The population pharmacodynamic parameters (PopPD) were determined through 60 different modeling and simulation evaluations using Monolix- 2016R1; by which the dose / response relationship was established for SBP, DBP and pulse) It was determined that the best structural model PopPD was referred to as Mlxt: immed. Emax _null, constant (-2 log likelihood = 447.20 for the systolic pressure, 449.81 for the diastolic pressure and 448.46 for the pulse). The parameters that were obtained were the average effective concentration (EC50) and the maximum effect (Emax). The evaluation of the model with covariates determined that age, schooling, cholesterol, triglycerides influence SBP; height, weight, body mass index (BMI), diabetes modify on the PAD and; the size, triglycerides affect the pulse.
The aforementioned covariates influenced the determination of the EC50 whose value was set at 115, 110 and 130 mg/L for SBP, DBP and pulse respectively; but it did not present a significant impact on the Emax parameter. |
Description: | El objetivo del presente estudio fue obtener un modelo farmacodinámico poblacional del losartán en pacientes hipertensos atendidos en el consultorio del Gobierno Autónomo Descentralizado (GAD) Municipal de Gualaceo, mediante modelos de efectos mixtos, asociándolos con las covariables de cada paciente.
Se evaluaron 50 pacientes hipertensos que cumplieron con los criterios de inclusión y exclusión establecidos. Se obtuvieron los valores de presión arterial sistólica (PAS), diastólica (PAD) y pulso; antes y después de la administración de losartán en forma aleatoria, dentro de un intervalo comprendido entre 1, 2, 3 hasta 6 horas post-dosis. Además, se recopiló toda la información pertinente como su historial clínico y medidas antropométricas. Los parámetros farmacodinámicos poblacionales (PopPD) se determinaron mediante 60 diferentes evaluaciones de modelación y simulación usando Monolix- 2016R1; mediante el cual se estableció la relación dosis/respuesta para PAS, PAD y pulso. Se determinó que el mejor modelo estructural PopPD fue el referido como Mlxt:immed. Emax _null, constant (-2 log verosimilitud= 447,20 para la presión sistólica; 449,81 para la presión diastólica y 448,46 para el pulso). Los parámetros que se obtuvieron fueron la concentración efectiva media (EC50) y el efecto máximo (Emax). La evaluación del modelo con covariables determinó que la edad, escolaridad, colesterol, triglicéridos influyen sobre la PAS; la talla, peso, índice de masa corporal (IMC), diabetes modifican sobre la PAD y; la talla, triglicéridos afectan el pulso.Las covariables mencionadas anteriormente influyeron en la determinación del EC50 cuyo valor fue fijado en 115, 110 y 130 mg/L para la presión arterial sistólica, diastólica y pulso respectivamente; mas no presentaron impacto significativo sobre el parámetro Emáx |
