| Abstract: | Introduction: Up to 25% of the diabetic population develop at least one foot ulcer throughout their life. The outcome is amputation, resulting in a physical limitation, recurrent hospitalization and a strong psychological damage to the patient.
Objective: To determine the sociodemographic, laboratory and clinical characteristics of patients with lower limb amputation attributed to diabetic foot at José Carrasco Arteaga Hospital from January 2013 to August 2016.
Methods: A descriptive study was carried out in patients from the José Carrasco Arteaga Hospital who had lower limb amputation per diabetic foot from 2013 to 2016. It was performed using an observational method of clinical histories and a descriptive analysis was performed.
Results: We found 68 patients amputated per diabetic foot (34.69%). A mean HbA1c of 9.84 (± 2.81) was found and only 20.3% reached the glycemic control target. A low percentage of patients with high levels of HDL (4.7%), LDL (4.7%) and triglycerides (3.1%) were found. 35.9% of patients had High Arterial Stress. The time to first amputation was greater than or equal to 21 years after diagnosis of diabetes. 47.4% had a sequential amputation in the following 3 years after their first surgery.
Conclusions: Patients suffering from a first diabetic foot amputation constitute a vulnerable group due to the advanced age and long evolution of the disease. The level of HbA1c found is indicative of poor metabolic control of the disease |
| Description: | Introducción: Hasta el 25% de la población diabética desarrollan al menos una úlcera en el pie a lo largo de su vida. El desenlace es la amputación, provocando de esta manera una limitación física, ingreso hospitalario recurrente y un fuerte daño psicológico al paciente.
Objetivo: Determinar las características sociodemográficas, de laboratorio y clínicas de pacientes con amputación de extremidad inferior atribuido a pie diabético en el Hospital José Carrasco Arteaga de Enero 2013 – Agosto 2016.
Métodos: Se realizó un estudio descriptivo en pacientes del Hospital José Carrasco Arteaga que tuvieron una amputación de extremidad inferior por pie diabético del 2013 al 2016. Se ejecutó mediante método observacional de historias clínicas y se realizó el análisis descriptivo.
Resultados: Se encontró 68 pacientes amputados por pie diabético (34,69%). Se halló una media de HbA1c de 9,84 (± 2,81) y solo el 20,3% alcanzó el objetivo de control glucémico. Se encontró un porcentaje bajo de pacientes que presentaron niveles altos de HDL (4,7%), LDL (4,7%) y triglicéridos (3,1%). El 35,9% de pacientes presentaron Tensión Arterial Alta. El tiempo de evolución hasta la primera amputación fue mayor o igual a 21 años tras el diagnóstico de la diabetes. El 47,4% tuvo una amputación secuencial en los siguientes 3 años después de su primera cirugía.
Conclusiones: Los pacientes que sufren una primera amputación por pie diabético constituyen un grupo vulnerable por presentar una edad avanzada y una larga evolución de la enfermedad. El nivel de HbA1c encontrado es indicativo de mal control metabólico de la enfermedad |
