| Abstract: | Objective
Determine the percentage of pregnant women with severe preeclampsia that have subtherapetic magnesemia after the administration of 4 grams as impregnation and 1 gram/hour as maintenance of magnesium sulftate, in the service of Obstetrics of the Hospital Vicente Corral Moscoso, Cuenca-Ecuador during 2014.
Materials and methods
Descriptive study involved all women with severe pre-eclampsia who received magnesium sulfate according with Zuspan scheme to prevent eclampsia and signed informed consent. Patients with convulsions and kidney or liver failure were excluded. Data were collected through a questionnaire about socio-demographic variables, medical history, laboratory tests and signs of intoxication at 4 and 24 hours after initiation of treatment. The analysis was done using the program SPSS version 15.
Results
108 cases were analyzed, there were no missing data. The average magnesium levels were 4.47 ± 1.69 mg/dl at 4 hours and 4.34 ± 1.54 mg/dl at 24 hours. The 69.4% had subtherapeutic levels at 4 hours and the 75.9% at 24. No patient had seizures. There was a case of intoxication that improved after the temporary de-crease of maintenance dose. Fisher test showed a significant difference between the magnesemia at 4 hours and nutritional status, p = 0.023; but not at 24 hours, p = 0.101.
Conclusion
69.4% of women in this study after 4 hours and 75.9% at 24 hours showed sub-therapeutic levels of magnesium |

Determinar el porcentaje de embarazadas con preeclampsia severa que presentan magnesemia subterapéutica después de la administración de 4 gramos de impregnación y 1 gramo/hora de mantenimiento de sulfato de magnesio, en el servicio de Obstetricia del Hospital Vicente Corral Moscoso, Cuenca-Ecuador durante el 2014.
Materiales y Métodos
Estudio descriptivo, participaron todas las mujeres con preeclampsia severa que recibieron sulfato de magnesio según el esquema de Zuspan, para prevenir la eclampsia y firmaron el consentimiento informado. Se excluyó a las pacientes con convulsiones e insuficiencia renal o hepática. Se recogieron los datos mediante un formulario sobre las variables socio-demográficas, antecedentes personales, exámenes de laboratorio y signos de intoxicación a las 4 y 24 horas del inicio del tratamiento. El análisis se hizo mediante el programa SPSS versión 15.
Resultados
Se analizaron 108 casos, no existieron datos perdidos. La media de la magnese-mia a las 4 horas fue 4,47 ± 1,69 mg/dl y 4,34 ±1,54 mg/dl a las 24 horas. El 69,4% presentó niveles subterapéuticos a las 4 horas y el 75,9% a las 24. Ningún caso presentó convulsiones. Hubo un caso de intoxicación que mejoró tras la dis-minución temporal de la dosis de mantenimiento. La prueba de Fisher evidenció una diferencia significativa entre la magnesemia a las 4 horas y el estado nutricional, p=0,023; no así a las 24 horas, p=0,101.
Conclusión
El 69,4% de mujeres del presente estudio a las 4 horas y 75,9% a las 24 horas presentaron niveles de magnesio subterapéutico |
