| Abstract: | Objective
To identify the metabolic status and associated factors in diabetic patients type 2, who were treated at outpatient in the Hospital of Girón, 2014.
Materials and methods
A cross-sectional study was made. The universe was formed with all type 2 diabetics who attended outpatient in the Hospital Aida León in Girón, in the months of June to September 2014. Data were collected by direct interview, anthropometric measurement and laboratory tests. Data were analyzed using the SPSS 22 software.
Results
About 115 patients were studied with an average age of 65.6 years, 54.8% women, 77.4% rural area, the average time to diagnosis 9.8 years, average of glycosylated hemoglobin 8%, glucose 175.6 mg / dl, total cholesterol 177,7mg / dl, HDL (high density lipoprotein) 42.9 mg / dl, LDL (low density lipoprotein) 111.4 mg / dl, triglycerides 181.7 mg / dl. systolic blood pressure 119,7 and diastolic blood pressure 70.4 mmHg, body mass index 28.69 Kg / m2 and waist circumference 97.4 cm. The 38.3% were on diet, physical activity 28.7%, 51.3% drug treatment; 73.1% used oral medication, attended educational workshops 45.2%.
Hypertension history (high blood pressure) 36.5%, dyslipidemia at the time of the study 91.3%. In terms of achievement ot the established goals was observed that 48.7% had adequate hemoglobin glycosylated, there was a good control for total cholesterol, HDL cholesterol and blood pressure that exceeded 50%.
The poor metabolic control was associated with age over 65 years (OR [odds ratio] 4,274 CI [confidence interval] 95% from 1.94-9.42 p = 0.000), failure to comply with the diet (OR 4.38 95% CI 1.95-9.86 p = 0.002) and do not fulfill the drug treatment (OR 2.43 95% CI 1.14- 5.14 p = 0.019).
Conclusion
The 48.7% had adequate glycemic control, the lack of control was associated with higher age to 65 years, noncompliance with diet and drug therapy. |

Materiales y métodos
Estudio transversal. El universo, todos los diabéticos tipo 2, consulta externa Hospital Aida León, cantón Girón, junio a septiembre del 2014. La información se recolectó por encuesta, medición antropométrica y pruebas de laboratorio, los datos se analizaron con el software SPSS 22, medidas utilizadas: porcentajes, media, OR, intervalo de confianza, el valor p.
Resultados
Se estudiaron 115 pacientes, edad promedio 65,6 años, mujeres 54,8%, zona rural 77,4%, tiempo promedio de diagnóstico 9,8 años, promedio de hemoglobina glicosilada 8%, glucosa 175,6 mg/dl, colesterol total 177,7mg/dl HDL colesterol 42,9 mg/dl, LDL colesterol 111,4 mg/dl, triglicéridos 181,7 mg/dl, tensión arterial sistólica 119,7 y tensión arterial diastólica 70,4 mmHg. El índice de masa corporal fue 28,69 Kg/m2 y perímetro abdominal 97,4 cm, cumplieron dieta 38,3%, actividad física 28,7%, tratamiento medicamentoso 51,3%; utilizó medicación oral 73,1%, asistieron a talleres educativos 45,2%. Antecedente de hipertensión arterial 36,5%, dislipidemia al momento del estudio el 91,3%. En cuanto al cumplimiento de las metas establecidas se observó que 48,7% tuvo adecuada hemoglobina glicosilada, más del 50% de pacientes tuvieron buen control para el colesterol total, HDL colesterol y para la tensión arterial.
El mal control metabólico se asoció con la edad mayor de 65 años (OR= 4,274 IC 95% 1,94-9,42 p= 0,000), el no cumplir la dieta (OR= 4,38 IC95% 1,95-9,86 p= 0,002) y el no cumplir el tratamiento medicamentoso (OR= 2,43 IC 95% 1,14-5,14 p=0.019).
Conclusión
El 48,7% tuvo adecuado control glicémico, el descontrol estuvo relacionado con la edad mayor a 65 años, el no cumplimiento de la dieta y del tratamiento medicamentoso. |
