En el caso de pacientes sometidos a maxilectomías, los implantes subperiósticos personalizados emergen como una alternativa efectiva a los implantes endoóseos convencionales. Objetivo: El presente artículo tiene como objetivo describir la rehabilitación mediante implantes subperiósticos personalizados de un paciente con antecedentes de maxilectomía parcial. Presentación de caso clínico: Paciente masculino de 77 años de edad que, tras una maxilectomía parcial y reconstrucción con colgajo libre escapular, fue rehabilitado utilizando implantes subperiósticos personalizados. Los implantes fueron diseñados digitalmente mediante un software de diseño asistido por ordenador (CAD), y se fabricaron utilizando titanio. La intervención quirúrgica se realizó con anestesia general, con incisión crestal y uso de una guía de posicionamiento para asegurar la colocación precisa de los implantes. Se colocó una prótesis fija provisional de polimetilmetacrilato inmediatamente tras la cirugía. Conclusión: Los implantes subperiósticos personalizados son una solución eficaz para defectos maxilares graves, con mejores resultados y tiempos de tratamiento más cortos gracias a los avances tecnológicos en su diseño.Introduction: Surgical reconstruction and subsequent rehabilitation in patients with severe atrophies or significant bone defects in the maxilla represent a complex challenge in maxillofacial surgery. In the case of patients who have undergone maxillectomies, customized subperiosteal implants emerge as an effective alternative to conventional endosseous implants. Objective: This article aims to describe the rehabilitation of a patient with a history of partial maxillectomy using customized subperiosteal implants. Case report: A 77-year-old male patient, who had undergone a partial maxillectomy and reconstruction with a scapular free flap, was rehabilitated using customized subperiosteal implants. The implants were digitally designed using computer-aided design (CAD) software and manufactured from titanium. The surgical intervention was performed under general anesthesia, with a crestal incision and the use of a positioning guide to ensure precise placement of the implants. A provisional fixed polymethyl methacrylate (PMMA) prosthesis was placed immediately after surgery.
