    Características del síndrome de burnout en docentes de la Facultad de Ciencias Médicas de la Universidad de Cuenca, Cuenca – Azuay 2024

    Background: Burnout describes a state of physical, emotional and mental exhaustion caused by chronic stress and the mechanization of work. Nowadays, in the context of a competitive society, the workload has increased and with it the frequency of Burnout Syndrome, which generates a growing concern about its effects on health and job performance. Objective: To determine the characteristics of Burnout Syndrome in teachers of the Faculty of Medical Sciences of the University of Cuenca, 2024. Methods: It is a descriptive study, the sample consisted of 260 teachers of the Faculty of Medical Sciences of the University of Cuenca, the data collection instrument was a form where sociodemographic data and the Maslach Burnout Inventory test were recorded. Results: The majority of teachers presented a low level of emotional exhaustion (74.4%) and depersonalization (68.7%), while 53.1% showed a high level of personal accomplishment. Teachers aged 30 to 49 years showed a higher prevalence of burnout, especially at moderate and severe levels. Women were the most affected group, with higher percentages of moderate and severe burnout compared to men. Conclusions: Family support and adequate relationships with coworkers are protective factors against burnout, while high workload and perception of inadequate remuneration increase the risk. These results highlight the need to implement intervention programs to improve teachers' well-being.

    Eficacia y seguridad del sulfato de magnesio como coadyuvante en la analgesia postoperatoria de histerectomía abdominal, bajo anestesia general balanceada. Hospitales Vicente Corral Moscoso y José Carrasco Arteaga. Cuenca. 2013

    Background: this becomes a pressing problem in Ecuador, where there are still inconclusive aspects to be considered. Objective: to determine the effectiveness and safety of intravenous magnesium sulfate as an coadyuvant to postoperative analgesia for abdominal hysterectomy under general anesthetics. Vicente Corral Moscoso Hospital and Hospital José Carrasco Arteaga Cuenca, 2013. Material and Methods: a clinical, randomized, single-blind study was performed; it was formed by two equal groups of 78 patients that were scheduled for abdominal hysterectomy. Group "A" was administered 50mg / kg Magnesium sulphate, and 15 minutes of balanced general anesthesia. In addition, analgesics such as tramadol 2 mg / kg at the time of aponeurotic closure; after that 1 mg / kg was first given every 6 hours during postoperative care and after that ketorolac 30 mg trans-surgical.; while "Group B" was given balanced general anesthesia using the before mentioned parameters with the exception of administering magnesium sulfate. Pain intensity was measured by visual analog scales. Results: group A showed mild pain average 3.76 + 1.22 compared to the group B which recorded moderate pain average 5.05 + 1.42; this difference is statistically significant up to 4 hours postoperatively with p = 0.000. No group reported cases of anaphylactic reaction, hypotension; nausea, vomiting or respiratory depression: bradycardia occurs in the two groups, having no significant statistical difference. Conclusions: magnesium sulphate proved effective as adjunctive analgesia until 4 hours postoperatively, its administration is safe

    Frecuencia y caracterización del intento autolítico en pacientes atendidos en el Hospital de Especialidades José Carrasco Arteaga Cuenca 2019 – 2021

    Autolytic attempts were examined in patients treated at José Carrasco Arteaga Hospital in Cuenca during the 2019-2020 period. It was found that 8.2% of patients attempted suicide, while 91.8% had suicidal ideation. Most cases occurred in female patients (67.8%). However, it is noted that this gender difference should be interpreted cautiously due to the high proportion of women in the sample. Young adults (37.8%) showed a higher propensity for autolytic attempts, which were associated with acute stress and medication ingestion. Additionally, complications related to psychological disorders were observed in autolytic attempts. This study supports the findings of the WHO, confirming a higher frequency of suicidal ideation compared to actual attempts, with a higher prevalence in young adults, rather than being specifically linked to the female gender.

    Prevalencia de enfermedad arterial periférica y factores asociados en clubes de pacientes con diabetes mellitus tipo 2, Cuenca 2014

    Objective: To determine the peripheral arterial disease prevalence and its related factors in type 2 diabetes people from the primary care diabetic patients clubs from Ministry of Public Health in the urban area from Cuenca city, 2014. Methods: Cross-sectional study in a calculated sample with an infinite universe, based on the 95% confidence inference with an error of 4.5% and lower expected prevalence of 10%. Diagnosis established by determining the ankle brachial index with a bidirectional vascular doppler examination with an 8 MHz probe EZ8 or VP5HS 5 MHz probe for edematous extremities, with the technique from American Heart Association 2012. Results: 254 patients were included with a mean age of 63.6 ± 11.4 years, predominantly female (70.3 %). The prevalence of peripheral arterial disease was estimated to be 47.2; 95% CI 41.1 - 53. 3 with significant association for a diabetes mellitus evolution of ≥ 5 years (OR 2.06; 95% CI 1.15 - 3.68, p = 0.015). 58.6 % of the population had a concomitant diagnosis of hypertension. The lipid profile values were close to the recommended ranges, the Glycosylated Hemoglobin A1c average value was 9.2 ± 2.7 %. Conclusions: The estimated prevalence of peripheral arterial disease is similar to that reported Studies with the ankle brachial index as a diagnostic criteria and was associated statistically with a diabetes duration for 5 years or more.

    Prevalencia del síndrome burnout y factores de riesgo en internos de medicina de la Universidad de Cuenca pertenecientes a la cohorte mayo 2022 – abril 2023. Cuenca, Azuay

    Background: Burnout Syndrome is a psychological disorder characterized by emotional exhaustion, depersonalization, and reduced personal accomplishment. The worldwide prevalence of the syndrome among health professionals ranges from 2.2% to 69.2%. It has represented a strong scientific interest in recent years by the medical community due to the high labor demand. Objectives: To determine the prevalence of Burnout syndrome and its risk factors in medical interns of the May 2022-April 2023 cohort of the University of Cuenca. Cuenca, Azuay. Methods: It was a cross-sectional analytical study. The sample consisted of all medical interns from the May 2022-April 2023 cohort. The method was the application of surveys where sociodemographic data, risk factors associated with Burnout Syndrome, and the Maslach Burnout Inventory test (MBI) were collected. The data was stored in the SPSSV24 digital database and analyzed according to the type of variable using the respective statistics. Results: The predominant age group was between 25-27 years. 63.5% of participants were women. 86.5% suffer from Burnout syndrome, and 48.6% suffer from it in its severe form. There was no statistical association between the variables studied and having Burnout syndrome. (p>0.005). Conclusions: Undergraduate medical interns presented high levels of risk factors associated with Burnout syndrome, its prevalence was high in this study

    Tamaño de las fontanelas anterior, posterior y factores asociados en recién nacidos a término del área de maternidad del Hospital Regional “Vicente Corral Moscoso”. Cuenca, 2013

    Introduction: the anterior and posterior fontanelles of the skull of newborn presents variations in size, with associated factors (gestational age, number of gestation, type of delivery), as is shown by studies where observed ranges from 1 - 7 cm in the anterior fontanelle, and 2-5 cm at the posterior fontanelle, the latter being closed at 49% of cases (5,6), relating changes in their sizes mainly neurodevelopmental disorders. Objective: To determine the size of the anterior and posterior fontanelles and associated factors in term infants, suitable for healthy, gestational age from 37-41 weeks of gestation, head circumference of 33-37 cm, of the maternity area in the "Vicente Corral Moscoso" Hospital in the city of Cuenca, 2013. Method: quantitative, cross-sectional study. The study population included infants of the Maternity area of "Vicente Corral Moscoso" Hospital, with sample of 272 newborn. The information was collected using a test developed by the authors; for data analysis SPSS version 15.0 software was used, with percentage, median, standard deviation, RP (prevalence ratio) with 95% IC and value of p. Results: The mean size of anterior fontanelle is found 2.28 cm, with a standard deviation of 0.74cm and 0.28 cm posterior fontanelle, with a standard deviation of 0.31 cm, similar to those reported in studies cited measurements, statistically significant association was found significant factors such as cesarean delivery, primigravidez and mixed ethnicity.
