Item Complicaciones por encamamiento prolongado en los pacientes del Área de Clínica y Cirugía del Hospital Vicente Corral Moscoso Cuenca 2014(Universidad de Cuenca, 2015-11-11) Cobos Dumas, María Valentina; Cruz Montaño, Estefanía Catalina; Paredes Muñoz, Lilia del Carmen; Zhapán Sisalima, Elizabeth Fernanda; Freire Argudo, Marco VinicioThe objective of this observational, descriptive and prospective study is to describe the complications due to prolonged lying times in patients who were hospitalized in the clinic and surgery area in Vicente Corral Moscoso Hospital, during the period of September 2014 to January 2015, were included all the patients found in prolonged lying times (˃ 72 hours) independently of their based pathology. To determine which patients were found in prolonged lying times the Karnofsky and Eastern Cooperative Oncology Group (ECOG) scales were used to interpret the functional state of the patients according to the World Health Organization. The data collection of each patient was made daily (annex #2) with the objective of registering the complications shown due to prolonged lying times and mortality during the hospitalization period. Of 1,369 patients admitted during our study of the surgery and clinic area, 150 were kept in prolonged bed rest, of which 81 corresponding to 54% had at least one complication. The most common complication was pressure ulcers with a percentage of 46.7%, followed by 10.7% nosocomial pneumonia and deep vein thrombosis with 4%. 69% of patients with complications from prolonged bed rest were older than 70 years. The diseases of the circulatory system CIE10 I00-I99 were more related to the presence of complications with a percentage 37.04%. 42% of bedridden patients who died were hospitalized between 3-10 days. In the results obtained from the present study we observed a high percentage of complications in patients on prolonged bed rest. This could prolong hospital stay and increase patient mortality, as well as the morbidity rate.Item Dosis de efectividad de radiación utilizada en estudios de cráneo, cuello, tórax y abdomen con un tomógrafo multicorte de 64 canales en el Hospital José Carrasco Arteaga Cuenca, septiembre del 2014-febrero del 2015(2015) Montenegro Córdova, Andrea Cristina; Toalón Idrovo, Henry Paúl; Ortiz Sagba, Nelson Ramiro; Freire Argudo, Marco VinicioComputed tomography allowing TCM has great advances in noninvasive diagnosis of various diseases. At present most of the applied radiation arises from the use of CT, followed by nuclear medicine and fluoroscopy. The aim of our research was to determine the effective dose in studies of skull, neck, thorax and abdomen with a 64-channel Multislice Tomography in Arteaga's José Carrasco Hospital in Cuenca Cato, during the period from September to February 2014 -2015. 3129 cases with a mean age of 48.58 years with the largest age group in patients 16 years and older with 90.3% were studied; the most common female sex was 50.5%. 4 types of tomographic methods were studied, being the most frequent with 68.1% skull and neck less frequent with 1%. The maximum effective dose used was 21,98mSv in the group of 16 years or more and the minimum 0,38mSv in studies of the skull of the same group. The effective dose in all CT without contrast in the group aged 16 and over did not exceed baseline levels. The value of the effective dose in all CT with contrast in the group aged 16 years and over exceeded baseline levels. Because no reference levels effective dose for pediatric groups found, the DLP was studied found higher levels in the first age group. The results obtained in this research will be disseminated in the Journal of the Faculty of Medicine of the University of Cuenca and study serve as a basis for future comparisons.Item Percepciones sobre la salud sexual y reproductiva de las mujeres indígenas Kichwas y Shuaras. Coverage of preventive screening is influenced by cultural con-ceptions of the population, which are based on the per-ceptions of each ethnic group on the etiologic process health-disease complex.OBJECTIVE: To interpret the perceptions of women of in-digenous peoples and nationalities Shuar of Sevilla Don Bosco and Kichwas of Quilloac and Saraguro, about sexual and reproductive health, sexually transmitted in-fections, cervical cancer and associated conditions. METHODOLOGY: A qualitative, phenomenological study was conducted through 17 focus groups and 13 stories of life study. A total of 102 people interviewed. RESULTS: The stories of women understood as collective constructions of their respective ethnic groups in the context of a Western, patriarchal and maleness hege-monic culture, mainly refer to destructive processes of sexual and reproductive health, manifested as infideli-ty and male promiscuity and as the lack of protection with condoms, which affects the prevalence of sexually transmitted infections and Cervical Cancer. In the parish of the pertaining Valley to the province of the Azuay Canton Cuenca, the percentage of mortality Hypertension is 58.4% and for Diabetes Mellitus is 41.6%. With the intention to maintain and/or decrease the values of blood pressure and blood sugar in adults and older adults in the Health Sub-Centre "The Valley" during the period 2014 – 2015 an exercise program was implemented through an investigation quasi-experimental, with whom he worked with a purposive sample, formed by people between 50 and 70 years of age with these pathologies and pharmacologically controlled than one year, those two measures were applied once before and once after the program physical exercises. In the initial evaluation values of blood pressure and blood sugar of 50 participants with the result that 21% suffer from hypertension male and 79% female is determined; while 19% suffer from NIDDM male and 81% female; according to what said was planned and executed a fitness program for four consecutive months; with the participation of 46 adults and seniors. At the end of the exercise program they managed to decrease the percentage of hypertensive patients from 46 to 26%, as did the percentage of diabetics 54 is decreased to 30%.
