    Características socio-demográficas asociadas a la presencia de dolor músculo-esqueletico, en Cuenca-Ecuador

    OBJECTIVE: To determine the socio-demographic characteristics associated with the presence of musculoskeletal pain (MSK) in people over 18 years who are living in Cuenca. PATIENTS AND METHODS An analytical cross-sectional study was conducted in subjects of rural and urban areas, who are over 18 years, using the methodology COPCORD (Community Oriented Program for the Control of Rheumatic Diseases) adapted to Ecuador. We identified individuals with rheumatic symptoms, house by house. The revision to confirm the diagnosis was made by a rheumatologist. The statistical analysis was univariate and multivariate. RESULTS A total of 2500 people were surveyed. The 59.6% were female, with an average age of 42.8 years, the 25.5% of participants performed domestic activities. The MSK pain occurred in 32.2% of people, which was significantly associated with living in rural areas, have lower education, lower income to $ 340, increased physical load of 8 pounds, more functional disability, and cooking with firewood in the rural area. CONCLUSION The MSK pain has a high prevalence in the population of Cuenca, Ecuador; producing disability especially in rural areas. It is associated with lower levels of education, lower income and increased physical load

    Calcium is a mineral most abundant in the human body and need an importants porcions. Serves multiple physiological funtions, the most important is the building are abone. Purpose: to assess calcium intake in womwn ages 15 to 30 years attending to “Vicente Corral Moscoso” and “José Carrasco” hospitals, Cuenca 2009. Methods: this work deeloped a descriptive estudy determine the calcium intake in women aged 19-30who attend the Vicente Corral Moscoso” and “José Carrasco” hospitals from Cuenca city basen use the nutricional satus usisg IMC and for socioeconomic status the un met basics need form valiated by the INEC Results: at the present study was obtained that 97.20% at the simple show a low calcium intake with average consumption of 410.66mg per day. We appreciate that this corresponds to the nutritional stats, tis74.50% has a normal IMC and 41.50% have a socioeconomic low down status. Conclusions: the study shows there are intake lower tan normal in both different sosioeconomics and nutritional status.

    Background: Obesity is currently a multifactorial public health problem. Its increase is directly related to comorbidities such as diabetes mellitus, insulin resistance, metabolic syndrome and others. Objective: To determine the factors associated with abdominal obesity in students in the Faculty of Medicine of the Universidad Católica, Cuenca 2014. Methodology: We conducted an observational, analytical cross-sectional study. We assessed a total of 300 faculty members. Nutritional status, the family history and the physical activity was evaluated through the use of validated test. To determine the association we used the statistical (Odds Ratio) with its 95% confidence interval. It was considered statistically significant results with a p value <0.05. Results: Of the 300 university students the average age was 21,57 years (SD: 2,59). The prevalence of abdominal obesity was 37% (IC95 %32 - 42), being highest in the age group of 25 a 36 years with 64,9%, as that was more prevalent in females 42,4%. Abdominal obesity of students surveyed is associated with age OR 0,268 (95% CI: 0,130 - 0,551), p = 0.000; with sex OR 1,77 (95%CI: 1,09 - 2,90), p = 0,021; and the sedentary lifestyle OR 2,037 (95%CI: 1,186 - 3,499 ), p = 0.009. Abdominal obesity is a risk factor for hypertension OR 2,797 (95%CI: 1,210 - 6,0), p=0,013. Conclusion: The prevalence of abdominal obesity is high and is associated with age, female sex and physical activity. And, is a risk factor for overweight, obesity and hypertension arterial.

    Factores asociados a pie diabético en pacientes con diabetes mellitus tipo 2 atendidos en el Hospital “José Carrasco Arteaga”, periodo febrero-mayo 2024

    Introduction: diabetic foot occurs in 8 to 12% of patients with diabetes mellitus and represents the main cause of non-traumatic amputation, as well as a public health problem. Objective: to describe the factors associated with diabetic foot in patients with type 2 diabetes mellitus treated at the “José Carrasco Arteaga” Hospital, February - May 2024. Methods: crosssectional analytical study in patients with type 2 diabetes mellitus attended in the outpatient and inpatient areas of the José Carrasco Arteaga Hospital. It was carried out by applying the “Structured form for diabetic foot detection and risk stratification”. The data were processed in SPSS 21 software (trial version). The results were presented in tables of frequencies and percentages, and OR with 95% CI and chi-square were used for association. Results: the prevalence of diabetic foot was 18.5%; the most frequent associated factors were: peripheral neuropathy (68.2%), lack of foot care (61.5%), presence of callous lesions (38.5%) and smoking (36.9%). A statistically significant association (p<0.001) was found between the factors: peripheral neuropathy, peripheral vascular disease, previous amputations, previous ulceration, significant structural foot deformity, callous lesion and glycosylated hemoglobin of more than 7.5% with the development of diabetic foot. Conclusions: it was evidenced that the most frequent factors associated with the development of diabetic foot were: peripheral neuropathy, lack of foot care, presence of callous lesions and smoking; while those that showed higher risk were: previous ulcers, previous amputations and peripheral neuropathy.

    Frecuencia de depresión y su relación con la capacidad funcional en pacientes con artritis reumatoide atendidos en el Hospital José Carrasco Arteaga de la ciudad de Cuenca, periodo: enero – mayo 2024

    Introduction: Rheumatoid arthritis is a chronic autoimmune disease present globally with a prevalence of 0.5-1%, it is directly and indirectly related to countless pathologies. Depression is the most common mental health disorder associated with rheumatoid arthritis, being twice as frequent in this group of patients than in the general population. The patient's functional capacity is affected by the chronic degenerative polyarticular involvement of the disease in close relationship with depression, causing the loss of the individual's physical function with limitations in the personal and work environment. Objectives: To determine the frequency of depression and its relationship with functional capacity in patients with rheumatoid arthritis treated at the José Carrasco Arteaga Hospital in the city of Cuenca, period: January-May 2024. Methods: Observational analytical cross-sectional study, using a sample of 185 patients. The Health Assessment Questionnaire was used to assess functional capacity and the Patient Health Questionnaire to assess depression. Results: According to the results of the PHQ-9, 15.5% of patients presented severe depression, and 22.2% presented a mild degree of depression. Regarding the HAQ, 48.45% presented moderate disability, while 10.82% presented severe disability. There is a high and positive relationship between the variables studied (r = 0.69).

    Ingesta de calcio en mujeres posmenopáusicas con osteoporosis que acuden a los hospitales Vicente Corral y José Carrasco, Cuenca, 2009

    Calcium is the most abundant mineral in the human body and is needed in large quantities because it has many properties, being better known for nourishing the bones. Objective: To determine and establish the intake of calcium in postmenopausal women with osteoporosis who come to hospitals Vicente Corral and José Carrasco of the city of Cuenca. Methods and Materials: We performed a descriptive study. The sample corresponds to 108 postmenopausal women with osteoporosis who go to hospitals "Vicente Corral and José Carrasco." Be evaluated calcium intake in food consumed daily by each patient by a survey of validated 24-hour recall for 3 days previously determined osteoporosis in postmenopausal women through an axial and femoral densitometry. For the statistical analysis used the software program and SPS FP2 and describe measures of central tendency, mean differences, percentages. Results: 97.32% of postmenopausal women with osteoporosis have a poor calcium intake. The average calcium intake was 530.18 mg / day, lower than that recommended by oms, while 1.79% of the respondents ate normal amounts of calcium and 0.89% of the people have a high intake. The 65.18% belonged to upper middle socioeconomic status and 46.43% had adequate nutritional status. Conclusion: In this present study, it has been a predominance of post menopause women with osteoporosis, with ingestion lower than 1200mg/day of calcium, in patients who used to go to the hospital “Vicente Corral” and “José Carrasco”.

    Validación del cuestionario COPCORD para detección de enfermedades reumáticas. Cuenca-Ecuador

    Objective: To validate the cultural adaptation of the instrument (Community Orientate Program in the Rheumatic Diseases) COPCORD to detect musculoskeletal pain and rheumatic diseases in the population of Cuenca. Methodology: A COPCORD transcultural questionnaire validation and its application were made in the community to identify rheumatic symptoms. A rheumatologist doctor checked people to define the diagnosis. A descriptive statistical analysis of each variable was performed. Chronbach alpha, analysis screening test (sensitivity, specificity, and positive predictive value and roc curves) was measured. Results: A total of 201 people participated, 106 in urban areas and 95 in rural areas. The 67.6% were women with an average age of 50.3 years with 7.5 years in school. The 57.7% reported musculoskeletal pain with an intensity of 5.8 for VSA. Comparing the COPCORD questionnaire with the clinical assessment of the rheumatologist an 83.72% of sensitivity, was observed, specificity of 61.14%, probability of 2.18 and area under the curve of 0.72; with a Cronbach’s alpha of 0.7 for pain characteristics and 0,9 for functional capacity. Conclusion: The COPCORD questionnaire is valid for using in rural and urban populations in Ecuador; it demonstrated good performance as a screening test.
