    Lumbocostovertebral syndrome. A case report

    Introduction. Lumbocostovertebral syndrome is a rare anomaly that affects the vertebral bodies, ribs and muscles of the abdominal wall, and can be associated with multiple congenital abnormalities. It requires multidisciplinary management and early surgical treatment to avoid complications. Clinical case. The case of a 10-day-old male neonate is reported, who presented from birth 2 right lumbar tumors, one that increased in size with crying, corresponded to a lumbar hernia, and the other, to myelomeningocele. In complementary studies, rib fusion, fusion of lumbar and sacral vertebral bodies (hemivertebrae), and abdominal wall defect with protrusion of intestinal contents were evidenced. The lumbar hernia was closed with prosthetic reinforcement with a bovine pericardium, without complications. Conclusion. In extensive defects, such as the one reported in this patient, it may be advisable to use prosthetic material. The bovine pericardium appears as a safe, well tolerated and effective option for these patients in par-ticular. This syndrome is a rare entity, which requires a multidisciplinary team for early surgical resolution and thus avoid complications.

    Manejo quirúrgico del secuestro pulmonar complicado. Informe de un caso.

    Introduction: pulmonary sequestration is a rare disease in adulthood; its highest incidence is in early stages of life. It is accompanied by repeated or severe chronic lung infections, and may be associated with congenital malformations. The diagnosis is based on the detection of anomalous irrigation through a vessel of the systemic circulation.This case allows us to propose a differential diagnosis in patients of adulthood, even when it is pathology with higher incidence in children and young people, and guide the management according to this diagnostic possibility.Clinical case: female patient, 19 years old, with a history of asthma and respiratory infections after a month of evolution. The patient was admitted with respiratory distress, fever and productive cough.A tomography and chest X-ray are performed; that show images compatible with collection in the left pulmonary field and pleural effusion. BAAR is performed in sputum with a negative result for tuberculosis.Posterolateral left thoracotomy is performed, left lower lobe resection, and pleural drain is placed. Pulmonary abscess is identified in the left lower lobe, and findings of anatomical pulmonary sequestration. Patient presents positive evolution; The clinical and radiographic control in the immediate and intermediate postoperative period was satisfactory.Conclusion: we present the case of adulthood, where an adequate diagnosis and multidisciplinary management allows a satisfactory evolution of the patient.

    Perfil epidemiológico clínico de neonatos sometidos a intervención quirúrgica en el Hospital Vicente Corral Moscoso, 2015-2017

    BACKGROUND: As the years have passed, care in neonates has improved, due to this a greater survival has been achieved with diseases that require surgical treatment. The World Health Organization indicates that each year 276,000 children worldwide worldwide die during the first month of life due to congenital anomalies. GENERAL OBJECTIVE: To determine the clinical epidemiological profile of the neonates operated surgically in the Regional Hospital "Vicente Corral Moscoso" during the period 2015-2017. METHODOLOGY: Descriptive and prospective study. The universe was constituted by the total number of patients surgically treated at the Hospital Vicente Corral Moscoso during the period 2015-2017, since it is an exploratory type study, no sample calculation will be done and the whole universe will be worked on. RESULTS: During the years 2015-2017 there were 83 surgically treated neonates of which the male sex was the most frequent 60.2%, the maternal age was between 20 to 29 years old with 48%. The majority were term neonates 71%. The most frequent condition was intestinal atresia 16.8%, the survival of newborns was 75.9%. CONCLUSIONS: The surgical newborn is a patient that requires a multidisciplinary confrontation due to its complexity. Perinatal care has evolved, achieving the survival of newborns with complex pathologies that require surgical treatment.

    Prevalencia de enfermedades hepatobiliares. Servicio pediatría hospital Vicente Corral Moscoso. Cuenca, 2017 al 2018

    Background: Some of the biggest challenges for pediatricians are liver diseases in children and adolescents because of their wide range of pathologies, subdivided into several others. Among the most prevalent diseases we can find: infant cholestasis (due to common bile duct cysts or bile duct atresia), acute liver failure, congenital liver fibrosis and also genetic or congenital diseases, such as metabolic defects, benign and malignant tumors, etc. General objective: to determine the prevalence of hepatobiliary disease in the pediatric service of Vicente Corral Moscoso Hospital, January 2017-December 2018. Methodology: a descriptive-cross-sectional study will be carried out in patients with a diagnosis of hepatobiliary disease in the pediatric service of Vicente Corral Moscoso Hospital, January 2017-December 2018. For data collection, a form will be applied, containing the different variables of this study. The information obtained will be processed through the SPSS V 15 program and the analysis will be carried out through descriptive statistics using frequencies, percentages and measures of central tendency (qualitative variables). The results will be presented in simple tables. Results: the prevalence of pancreatitis was 39.3%; liver failure 14.3%. The prevalence of hepatomegaly was 23.2%, while that of jaundice was 14.3%, and finally, the prevalence of cholestasis was 32.1%. Only 7.1% had bile duct atresia.
