    Análisis de la relación espacial del seno maxilar con el tercer molar superior erupcionado a través de tomografías computarizadas de haz cónico: Facultad de Odontología. Universidad de Cuenca. Periodo 2017-2024

    Introduction: The maxillary sinus is a paranasal structure located in the maxillary bone, it maintains a close anatomical relationship with the upper third molars. This proximity may increase the risk of surgical complications, such as sinus perforations or oroantral fistulas, especially during endodontic or surgical procedures. Cone beam computed tomography (CBCT) has been shown to be more accurate than panoramic radiographs in the three-dimensional evaluation of these anatomical relationships. Objective: Analyze the relationship and anatomical proximity between the apices of erupted upper third molars and the floor of the maxillary sinus, according to their vertical classification, by analyzing CBCT images. Materials and methods: Observational, descriptive, cross-sectional study with a quali-quantitative approach. We analyzed 105 CBCTs of patients between 21 and 59 years old, obtained between 2017 and 2024. The distance in millimeters between the nearest root apex and the floor of the maxillary sinus was measured, and the vertical relationship was classified as inside (IS), in contact (CO) or outside (OS). Results: The most frequent relationship was CO type (38.2%), followed by OS (36.8%) and IS (25%). Significant differences were found in the anatomical proximity between these groups, with higher surgical risk in CO and IS types. There was a slight association with sex and a higher prevalence of type CO in young patients. Conclusions: The use of CBCT allows an accurate assessment of the maxillary molar-sinus relationship, essential for surgical planning. Its routine use is recommended when two-dimensional images suggest a close or risky relationship.
