    Alteraciones radiológicas en la columna dorso lumbar en pacientes diagnosticadas de osteoporosis atendidas en los hospitales Vicente Corral Moscoso y José Carrasco Arteaga de la ciudad de Cuenca 2010-2011

    To determine radiological alterations in column dorsolumbar in patients diagnosed of osteoporosis between 50 and 65 years attended in the hospitals Vicente Corral Moscoso and Jose Carrasco Arteaga and to put in consideration doctors and public in general of his importance for the opportune prevention and treatment. MATERIAL AND METHODS.- In this study there take part 100 patients shortlisted with diagnosis of osteoporosis by means of densitometria, considering variables as: age, density, trabecularización, thickness cortical, vertebral saucers, vertebral height, vertebral deformities, degree of vertebral compression,- RESULTS 34% of the participants present some type of deformity to column level dorsolumbar: 21 persons present wedge, 10 biconcave, 2 combination of wedge and biconcave and 1 person presents crushing. CONCLUSIONS.- The study determines that the visualization of the vertebra has narrow relation with the variables aspect, trabecularizacion, aspect of trabeculas, vertebral saucers, thickness of corticals and height.- The most frequent age of vertebral deformity in the current study the deformities place in 60-61 years of age.

    Caracterización bacteriana en flora intestinal y pruebas de resistencia antibiótica en 500 escolares del sector urbano de Cuenca, año 2009

    Objective: Bacterial characterization in Intestinal Flora and Tests of Antibiotic Resistance in 500 scholars of the Urban Sector of Cuenca, Year 2009 .-Materials and Methods. With an Observational Descriptive design, 500 samples of fecal grounds were gathered of school and they were processed in the laboratory of microbiology of the Ability of Sciences you prescribe, during the period September of the 2009. - Results. -Of the 500 scholars 279 were women and 221 men, the percentage distribution for ages was: 5-7 years (33.6%), 8-10 years (44.4%) and of 11-13 years (22.0%). The carried out bacterial isolations showed to Escherichia coli in 80%, Enterobacter 13%, Klebsiella 5.8%, Proteus 0.8% and Pseudomona 0.4%. - It was determined that all the bacterias turned out to be sensitive in a high percentage to Imipenem 100%, Cloranfenicol 98.4%, Ciprofloxacina 97.5%, Cefotaxime 95.5%, Cefurexime 93.3%, and Ceftriaxona 90.2%. Escherichia coli presents resistance to Trimetoprinsulfa with 20%, Gentamicina 23.5% and Ampicilina+ Sulbactam 25.3%; as for the Enterobacter it was it for Gentamicina 15.4%, Ampicilina+Sulbactam 18.5%, in what concerns Klebsiella 24.1% point for Gentamicina and Ampicilina+ Sulbactam and Trimetoprinsulfa 27.6%. Proteus and Pseudomona didn't present resistance some. Conclusions. - Escherichia coli was the bacteria of more frequency contrary to Klebsiella, Enterobacter, Proteus and Pseudomona, the bacterial resistance was comparatively low.AU

    Caracterización y resistencia de proteus, pseudomona, klebsiella y enterobacter en 1000 cultivos primarios en pacientes de los hospitales Vicente Corral Moscoso y José Carrasco Arteaga, Cuenca, 2008-2009

    Objective. Characterize the sensitivity and resistance of Proteus, Pseudomonas, Enterobacter, Klebsiella isolated in crops in the Vicente Corral and Jose Carrasco hospitals of Cuenca city. Materials and Methods. With a cross sectional study was collected information 1000 primary cultures in the outpatient and hospitalization of both houses of health from 2008 to 2009. Results. In the 1000 primary cultivations 789 (79%) picked up in the Hospital Vicente Corral and 211 (21%) in the Hospital José Carrasco 137 were identified (13,7%) positive cultivations for: Klebsiella spp (5,5%), Enterobacter spp (4,9%), Proteus spp (1,7%) and Pseudomona spp (1,6%). In both hospitals the biggest percentage was of the Service of External Consultation. Pseudomona spp went more resistant to Cefepima (53,4%). Gentamicina (50%) and Ceftriaxona (50%); Klebsiella spp to Ampiciline Sulbactam (52,8%) and Gentamicina (40%); Enterobacter spp to Gentamicina (65,4%), and Proteus spp to Ciprofloxacina (41,2%) and Sulbactam (35,3%). The summary characterizes the variables of interest like age, sex and bacterial resistance in patient as well as the hospital areas where were assisted and collection the sample for cultivation and antibiograma. Technique was used settled down in cultivation microbiology (primary, secondary), you prove biochemical of identification and antibiograma of Kirby Bawer, validated internationally. Conclusions: Sensitivity and bacterial resistance patterns are very changing and differ markedly between health centres. Bacterial behavior seems to be determined by the abuse of antibiotics.     Factores asociados a enfermedad pulmonar obstructiva crónica, hospitales José Carrasco Arteaga y Vicente Corral Moscoso, 2011-2012

    Main objective: Our main objective is to determine risk factors which are associated to chronic pulmonary disease, among patients who are 40-85 years old, who were attended to the external consultation wards or admitted to the Internal Medicine and Pneumology Departments of the Jose Carrasco Arteaga and Vicente Corral Moscoso hospitals from 2011 to 2012. Materials and methods: This is a case-control research. Our sample was calculated taking in account a 95% confidence interval, 80% ratio of statiscal power, odds ratio of 3 and a 10% of exposition factor with its lowest frequency; a couple procedure was applied according to sex and age and were interned in a sequential manner. All data was obtained by a direct interview and was analyzed with the SPSS analytics software. Results: 318 patients were evaluated, of whom, 106 were cases and 212 controls. Both groups were similar in relation to age and sex (p>0.05). 72.3% were male, with an average of 62.4 years old (±13.1D.E.). The pack/year index superior to 20 was a risk factor for COPD (OR de 9.03, CI95%, 3.76 – 21.72; p = 0.000), equally biomass fumes exposure in years/hours index greater than a 100 (OR 9.65, CI 95% 4.87-19.32, p=0.000). Dust exposure, air pollution and family antecedents of COPD in patients’ parents were not risk factors for COPD. Conclusions: The risk factors for COPD are attributable to smoking and biomass fumes exposure.

    Factores asociados a hemorragia digestiva alta no variceal en pacientes mayores de 39 años: Hospital Vicente Corral Moscoso, Cuenca, 2012

    OBJECTIVE: To determine the association between non-variceal upper gastrointestinal bleeding with NSAIDs consumption, alcohol, snuff and personal history of dyspepsia in patients over 39 years, hospitalized in the Department of Internal Medicine Vicente Corral Moscoso during January 2008 to January 2012. MATERIALS AND METHODS: We conducted a case-control study. The sample was calculated based on 95% confidence, power of 80%, OR of 3 and 15% of consumption inflammatory and size formula was applied to cases and controls. Were matched by age and sex, and two controls per case. Data were obtained from medical records and direct interview and analyzed with Epi Info RESULTS: 60.1% were men. The age ranged between 40 and 94 years, the median was 73.0. The groups were similar in mean age and frequency of female and / or male (p> 0.005). Consumption of NSAIDs, snuff and personal history of dyspepsia were are risk factors for upper gastrointestinal bleeding (OR: 2.1, 95% CI 1.2 - 3.6 and p value = 0.006) (OR: 2.0, 95% 1.1 - 3.6 and p value = 0.018) (OR: 22.4, 95% CI 9.2-54.7 and p value = 0.000) respectively. CONCLUSIONS: Consumption of NSAIDs, snuff and personal history of dyspepsia are risk factors for non-variceal upper gastrointestinal bleeding.

    Factores de riesgo asociados a cáncer gástrico en pacientes diagnosticados desde 2000 al 2010 en el Instituto del Cancer SOLCA, Cuenca

    Objective. Identify the characteristics of gastric carcinoma and identify associated factors in a population of patients diagnosed and treated for more than ten years years in the Cancer Institute Hospital of Cuenca city. Material and methods. Using a descriptive design, we collected 600 medical records belonging to the same number of patients whose diagnosis was confirmed by histology. Results. The average age was 50.6 ± 10.7 years (23 - 83). The cancer was more common on the fifth decade. 42.5% of the group had 50 to 64 years of age. Males were most affected (61%). Chronic atrophic gastritis was 50.5% and 41.2% in gastritis. The Helicobacter pylori was found in 65.7%. 29.7% and 29.5% of patients had alcoholism and smoking. Heredity were found in 40.7% of patients (n = 244). Of these, the history of the disease in parents was significantly higher (P = 0.012) and was found in one in seven patients (16.3%). Discussion. The results of our study are similar to those reported in the literature. The role of Helicobacter pylori and heritage have a growing role in the genesis of gastric carcinoma.

    Background: The Infection of the Tract Urinal is a very frequent pathology in our patients, therefore it is important to know the causal agents of this pathology that it is the Enterococo in our study. Objectives: To know the frequency of the Enterococo in the Infection of the Tract Urinal and their sensibility to the antimicrobianos. Patient and methods: He/she was carried out a study of descriptive type in 400 patients of the Hospital Vicente Corral Moscoso in who were carried out urocultivos and antibiogramas of the positive one for Enterococo, determining the sensibility and resistance from this patógeno to the used antimicrobianos. You gathers the such patient's information as age, sex, residence, and hospital service of where the same one comes. Results: the frequency of the enterococo like causal agent of the infection of the tract urinal was of 15.3%. you/he/she Showed a bigger frequency in the patients of feminine sex and those that came from external consultation and of the urban area. He/she was resistance of Enterococo in 57.38%, 45.95% and 36.07% for Cefalexina, Ciprofloxacino and Doxiciclina respectively. Conclusions: The enterococo continues being frequent cause of Infection of the Tract Urinal. He/she was an increment in the percentages of resistance of the Enterococo for the antibiotics more commonly employees.     Hallazgos clínicos, endoscópicos altos e histológicos de pacientes con síntomas dispépticos atendidos en el Hospital José Carrasco Arteaga durante 2010-2011

    Introduction: Dyspepsia is a common symptomatology in daily medical practice, affects notably patients’ quality of life.- The prevalence of clinically significant upper gastrointestinal findings in adult patients and their predictability is unknown.- Objective: To determine frequency of clinical, endoscopic and histological findings in patients with dyspeptic symptoms treated at Gastroenterology Unit of José Carrasco Arteaga Hospital in Cuenca from January 2010 to December 2011.- ethodology: A descriptive epidemiological study of cross section was conducted, from January 2010 to December 2011. The population consisted of all patients with dyspeptic symptoms who attended the service that met the inclusion and exclusion criteria established underwent upper gastrointestinal endoscopy, and samples were taken for histological research. The data were transcribed AS400 system to the respective forms, which were analyzed with SPSS 19.0.- Results: We reviewed 2795 patients’ records, the most common age group is between 46 and 60 years old (36.1%), predominantly female (60.4%), the most frequently reported symptom was epigastric pain (84.3 %), followed by abdominal distension (50.5%). Functional dyspepsia was the most frequent endoscopic finding (60.5%) versus organic dyspepsia (39.5%). The most frequent significant endoscopic findings were erosions (16.1%) and esophagitis (12.5%). Infection by H. pylori was present in 61.6%. The chronic active gastritis histological finding was observed more frequently (36.6%). Conclusion: Features observed among population of Cuenca city don’t differ too much from the international studies reviewed.AU

    Introduction: The main reservoir of Staphylococcus aureus is human nostrils. It is known that nasal carriers of Staphylococcus aureus are at increased risk of 3-6 fold greater risk to get infections from this germ. In its variety methicillin resistant shows itself as one of the most important hospital pathogens. Objective: To determine the prevalence of nasal carriage of Staphylococcus aureus in hospital personnel and their antimicrobial susceptibility pattern. Cuenca 2010. Method: It is a cross-sectional design carried out in Vicente Corral Moscoso Hospital and División III Ejército Tarqui. The studied population was composed by 150 people who work in intensive care units, neonatology wards and operation rooms. Samples were collected by nasal swab, transported in Stuart's medium, cultivated on blood agar and mannitol agar. We performed Gram stain, catalase test, coagulase, DNase and it was performed antimicrobial susceptibility testing by the disc diffusion method. We made a descriptive statistical analysis for a better understanding and interpretation of the data. Results: 55 strains of Staphylococcus aureus were isolated from the tested crops. The prevalence was 37%. The most colonized personnel was the neonatology staff with 39%. Most carrying was observed in medical personnel with 50%.They were susceptible to trimethoprim-sulfamethoxazole, and vancomycin 100%; cefoxitin 94,55; gentamicin 90,9%; clindamycin 89%; tetracycline 87,3%; erythromycin 49,1%; 18,2% penicillin. Conclusion: 37% of hospital staff was nasal carriers of S. aureus, being potential spreader. The antimicrobial susceptibility of this microorganisms decreased compared to methicillin.

    Parásitos en expendedores y hortalizas de los mercados públicos. Cuenca 2015

    Objective: determine the presence of parasites in sellers and vegetables of the public markets of CuencMaterials and methods: it is a transversal analytical study. The sample consisted of 144 sellers; it was made in four markets in Cuenca. Initially the merchants were informed about the characteristics of the investigation, the informed consent was requested, a survey was applied and a stool sample was requested, later a lettuce and some parsley were purchased for their respective analysis. The faecal matter was subjected to a coproparasitological examination and stool concentrate using the simplified Ritchie method, fresh technique with saline solution and lugol, and for the observation through a microscope; the vegetables were placed separately in a beaker with 400 ml of distilled water for 24 hours, later about 8 ml of this water was placed in a test tube and it was centrifuged, the sediment was observed with microscope. The processing of samples met the standards of quality and biosafety. The data were analyzed using the IBM SPSS Statistics 20 program and the Epi info 7 program (trial versions). Results: the presence of parasites in sellers is determined by 68.10%, the main protozoa observed are Entamoeba histolytica 19.03% cysts and 16.45% Cryptosporidium parvum oocytes. The parasite contamination in parsley was 44.40% and in lettuce 38.90%. In both products, there is the presence of filariform larvae of Uncinaria spp Conclusions: the presence of parasites in sellers and products is observed. The most frequent intestinal parasites are the cysts of Entamoeba coli and oocytes of Criptosporidium parvum. In the vegetables, the presence of the filariform larvae of Uncinaria spp is prominent.     Prácticas de estilo de vida de los adultos mayores diagnosticados de hipertensión arterial esencial en el Area 2 de Salud de Cuenca, 2011

    Objective Determine the lifestyle practices in elderly patients diagnosed with essential hypertension attending the Area 2 Health Cuenca, 2011. Material and Methods We conducted a descriptive study, a sample of 130 hypertensive patients from a total of 269. The sample was random, representative and weighted for each health center. The size was calculated on the basis of the known universe and a cross-sectional research that found a prevalence of smoking in the general population of 20%. Results The mean age was 73.8 ± 8.4 years, 75.4%, female, 93.1% had an education level between 1 and 6 years, 63.8% residing in urban areas and 44.6% were diagnosed hypertension the last 4 years. 41.5% were overweight, obesity 22.3% and 30%, poor control of hypertension. 81.5% were sedentary, 78.5% adding extra salt to meals, drank alcohol, 4.6% and 1.5% smoked. Conclusions Most were sedentary and added extra salt to meals and drank alcohol or smoked minority.

    Prevalencia de infecciones nosocomiales y factores de riesgo asociados en pacientes atendidos en el Hospital Vicente Corral Moscoso, Cuenca 2010

    Objective. To determine the prevalence of nosocomial infections (NI) and its associated factors in a hospital complexity level three in a period of one year. Material and metods. Using a cross-sectional design, we collected 400 records of patients of Vicente Corral Moscoso Teaching Hospital, Cuenca, who were hospitalized in 2012 in Clinical, Surgery, Pediatrics, Obstetrics, Neonatology, Obstetrics, ICU and Emergency. We analyzed the prevalence of NI and its associated factors were identified by calculation of the RP (95%) and the magnitude of the association using Pearson 2. Results. The prevalence of nosocomial was 19.8% and germs detected were cocci and Gram + and -. The most prevalent infections were sepsis (75.9%), urinary tract infection (11.3%) and pneumonia (10.1%). In Clinical Pediatrics and Neonatology is a higher prevalence of IN. Average age of the sample 52.4 ± 23.1 years (range 1 to 99), 54% were women. Average hospital stay 14.8 ± 12.7 days (range 3 to 90). Factors significantly associated (P <0.05). To the IN were: famale gender [OR 2.1 (95% CI 1.4 to 3.2)], age over 51 years [OR 2.1 (95% 1.4 to 3.2)], malnutrition [OR 1.7 (95% CI 1.1-2.6)], hospital stay> 15 days [OR 2.9 (95% CI 1.9 to 4.3) ], nasogastric tube [OR 19.4 (95% CI 8.1 to 47.0)], parenteral nutrition [OR 4.5 (95% CI 3.2 to 6.3)], antibiotics [OR 4.4 (95 % 2.1 to 9.4)], mechanical ventilation [OR 4.4 (95% CI 3.2 to 6.3)], endotracheal intubation [OR 3.2 (95% CI 2.2 to 4.7)] and blood transfusions [OR 2.8 (95% CI 1.9 to 4.1)]. Discussion. The rate of nosocomial infections in our hospital was higher than expected. The associated factors identified are similar to those reported in the literature.

    Prevalencia de portadores nasales de Staphylococcus aureus en el personal del Hospital Vicente Corral Moscoso y Hospital Militar, patrón de sensibilidad antimicrobiana. Cuenca, 2010

    Introduction: The thyroid gland is a hormone-producing organ that plays an important role in metabolism. The classification TIRADS (Thyroid Imaging Reporting and Data System) gathers ultrasound scan patterns to differentiate between benign and malignant nodules in a better way, this means a better selection of patients who need surgery and cytology studies. Objectives: To validate the ultrasound classification- TIRADS in patients with thyroid disease who undergo ultrasound for evaluation of thyroid gland in Cuenca, Ecuador. Materials and methods: This validation study of diagnostic test, analyzed the thyroid pathology through ultrasound scan in relation to cytology as a gold standard with thin needle aspiration puncture technique. Results: We studied 860 patients, 710 women and 150 men. Validation values in this study were: sensitivity 97,87%, specificity 72,32%, 30,26% positive predictive value, negative predictive value 99,64%, Youden test 0.70, positive likelihood ratio of 3.54 and negative likelihood ratio of 0.03. Conclusions: TIRADS classification is a highly sensitive method that can be used in imaging services in the studied hospitals.

    Prevalencia de portadores nasales de staphylococcus aureus, patrón de sensibilidad antimicrobiana y factores asociados en los cuidadores de adultos mayores en los Centros Geriátricos del distrito 6. Cuenca-2016

    Objective: determine the prevalence of nasopharyngeal carriers of Staphylococcus aureus, antimicrobial susceptibility pattern and associated factors in elderly caregivers in the geriatric centers of district 6 in the city of Cuenca during 2016. Material and methods: A cross-sectional study was conducted. The sample size was randomly calculated with 206 participants. The investigator performed the sampling of nasal passages, cultivating on blood agar and mannitol. We consulted on the associated factors, after the data collection were introduced on a basis in SPSS version 15. Results: in the study 89.9% were women. The age median was 35 years. The prevalence of nasal carriers of Staphylococcus aureus was 16% (95% CI: 11.2-21,8). The same had a sensitivity of 100% to vancomycin, cefuroxime and novobiocin. It was associated with diagnosis of chronic pathology (RP 2,813 95% CI: 1,467-5,396 p <0.001); use of daily medication (RP 2,944 95% CI: 1,576-5,498 p <0.001) and hospital visit (RP 2,377 95% CI: 1,126-5,017 p 0.016). Conclusions: The prevalence found differs from other studies reviewed and was statistically associated with diagnosis of chronic pathology, use of daily medication and hospital visit, the bacteria was sensitivity a vancomycin, cefuroxime and novobiocin similar to studies in Latin America and different from European countries
