Congenital anomalies are a group of disorders of variable etiology and
heterogeneous nature, present in 6% of births worldwide. The objective of this
study was to analyze the spatial distribution of congenital anomalies in children
under 1 year old, attended in health units in Ecuador, and their relationship with
residence in areas of mining, oil and agro-industrial exploitation, in the period
2015-2019.
A critical epidemiological study was carried out that allowed the analysis of
extractive contamination and agricultural production, and the presence of
malformations using the critical processes matrix, which is a tool for analyzing
the movement of social determination of health. The research used a
descriptive analysis, modeling and distribution of clusters of care records for
children under 1 year of age in units of the Ministry of Health, during the years
2015 to 2019, and a socio-environmental and geographical analysis of the
critical processes of environmental contamination in the equator.
The results show that the distribution of each group of congenital anomalies
varies between the provinces and cantons of Ecuador and increases in places
with greater extractive exploitation and industrial agricultural production.
Ecuador is a scenario where epidemiological and spatial expressions of the
capitalist model are manifested, which conditions critical socio-epidemiological
and environmental processes. The productive nature of exploitation is due to a
discriminatory and exclusive social order, which results in evident contrasts in
the levels of exposure and the consequent incidence of congenital defects in
the child population.

Background: leucorrhoea and vaginal pruritus are the main causes of gynecological consultation, are related to infections; timely and comprehensive diagnosis is the fundamental pillar to reduce recurrences that interfere with quality of life.
Material and methods: observational, analytical, cross-sectional study; sample: 430 Gynecology patients from the Centro de Especialidades Central Cuenca (IESS). Forms were applied, samples were taken: fresh, Gram, Papanicolaou and colposcopy. Descriptive analysis was carried out: frequencies, percentages, means, DS; the association was determined by Odds Ratio and confidence intervals (95%). Significant results were considered: values of p <0.05.
Results: sample conformed by women between 25-44 years (80%) (35.85 ± 7.5); urban residence: 71.2%; married: 57.2%; full upper instruction: 37.2%; multiparous 56%; mestizas: 99.5%; Typical average economic level (43%). The vaginal infection was 62.1% (267); Bacterial vaginosis (VB): 62.92%; candidiasis: 30.33%; trichomoniasis: 6.74%. Colposcopy altered in 66%, Nugent criteria: VB: 43.5%. Papanicolaou altered: 24.2%; Amsel criteria: VB: 21.9%. Start of sexual life between 12-18 years: 48.7% (average: 19.87 ± 4.2); sexual partners: between 2-4: 53% (mean: 2.19 ± 1.6); frequency of relations / week: 1 in 37.4% (average: 1.74 ± 1.2); previous diagnosis of sexually transmitted infection (STD): Human Papillomavirus (HPV) in 68.6%; they used planning methods: 49.1%; low socio-economic level: 13.2%; no significant relationship was found between the variables of possible risk and the presence of infections.
Conclusions: the prevalence of vaginal infections was high, especially Bacterial Vaginosis and there were no risk factors related to: BV, candidiasis or trichomoniasis

Objective: To evaluate the quality of outpatient care, through the measurement of perceived satisfaction with services received.
Methodology: A cross-sectional descriptive design was applied. The study was conducted at the Center for Health 2, Cuenca, July 2014, in a probabilistic sample of 287 users. SERVQHOS the validated questionnaire, adapted for research purposes was applied. The survey was administered to adult users, after care in outpatient services. The data analysis was performed using descriptive statistics, through the SPSS version 15.0 software. Discrete variables were operationalized in numbers and percentages and continuous variables as mean ± standard deviation (X ± SD). Wallis and chi square - statistical tests Cronbach's alpha (0.866) and Kruskal were applied
Results: The sample was composed of young adult users (35.1 ± 14,1 years), 76.3% women, married status (46.7%), urban residence (94.1%) of instruction half (46.7%), more frequent (35.9%) housework and uninsured (77.7%) occupation. The service was consulted Medicine (55.1%). High levels of satisfaction were recorded in all departments and globally (77.4%), with statistical significance (p<0.05). A 76.3% of subsequent visits. The high degree of satisfaction was statistically significant in subjective categories.
Conclusion: High levels of satisfaction with the overall and individual service is found, resulting in quality service. User satisfaction is associated with his treatment of health personnel.

Objective
Determine the home management of waste generated in the treatment and self-monitoring of glucose by patients diagnosed with diabetes attending outpatient Donum Foundation in Cuenca, during 2014.
Materials and methods
A descriptive study was conducted in 203 patients with diabetes who attended outpatient in the Donum Foundation between September and December 2014. Data were obtained with direct interview and analyzed using the SPSS software.
Results
Of the 203 diabetic patients the predominant age group is between 59 and 68 years (40.4%). Most women (67%). 98% of patients were diagnosed with type 2 diabetes millitus and the remainder with other types of diabetes.
Of the total patients interviewed 47.3% produce infectious waste in their homes and performing treatment and self-monitoring of glucose. This percentage: 97.4% does not separate the waste generated from other household waste; 97.9% of these wastes improperly stored indoors; and removes 100% of their homes improperly.
Conclusion
Of diabetic patients attending outpatient Donum Foundation in Cuenca, between September and December 2014, and infectious waste generated in their homes for treatment and self-monitoring of glucose, over 95% separated, stores and disposes of waste improperly

Nursing sees human care as a necessity, not only biological or physical, but that goes beyond a whole. Objective: to explore the social presentations of nursing in coping with drug use in adolescents of CETAD-Libertadores de Guayaquil 2022. Methodological design: qualitative-ethnographic study allowed to explore the social representations of nursing in coping with drug consumption. The subjects went to 9 nurses with experiences in the care of adolescent drug users. For data collection, focus groups were conducted, interviews were conducted following the Script of guiding questions, for validation the credibility method was used, making the transcription and interpretation of the data as close as possible to the reality studied. For the analysis of the data, the method of constant comparison was used, organized with the Atlas ti. 22. Software. Results: Social interaction nursing-patient and family in coping with drug use in adolescents, social visibility of nursing in coping with drug use in adolescents, Role of social accompaniment of nursing in coping with drug consumption in adolescents. Conclusions: The social representations of nursing in coping with drug use is based on guidance, commitment, and responsibility, as an individual process in the adolescent for coping. Objective: To determine the prevalence of hypertension and associated factors in adults between 44 and 64 years of age, in the parish Chiquintad. Cuenca. 2014.
Method: A cross-sectional study was conducted in a population sample of 275 people from 44-64 years of age. The information in the SPSS system Version 15 (Statistics for Social Sciences) was processed calculated based on the 95 % confidence, 20 % prevalence of hypertension, inference error of 5%.
Results: A study of 275 people: 84.4 % (232) of feminine gender, median age 54.25 years; 66.2 % were married, 14.5 % single; 59.3 % were engaged in household chores, 19.6 % in agriculture and 11.3 % at Artesian; 86.5 % had primary instruction.
Total prevalence of hypertension is 18.5 %, corresponding to the 14.9 % HTA I and HTA II at 3.6 %; Overweight 47.3 % and obesity 35.6 %, central obesity 61.8 % (170 people); 82.0 % are inactive or sedentary (253), 54.2% have a family history of hypertension.
We see that there is a statistically significant association with age (OR 0,324 with 95%: CI 0,168–0,626 and value p = 0.001); in addition to abdominal obesity (OR 2,037 and 95 %: between 1.028 and 4,036 with p = 0.039 value).
Also, 89.1 % and 77.1 % preferably consumed cereals and junk food respectively. The other food groups at high consumption by the population (without involving risk) with percentages above 50 %, except proteins (7.6 %), so you can ensure that this population largely consumes what they produce.
Findings: The prevalence of hypertension (18.5 %) in the parish of Chiquintad is associated with age, abdominal obesity.

Objective.- Describe the characteristics eating habits of adolescents in the eighth and ninth grade of Sayausí Javeriana College.
Specific objectives.-
• Determine size, age and body mass index (BMI) with data from sample and compare with growth patterns provided by the CDC / NCHS as an indicator of an adequate diet.
• Calculate the daily energy consumption, the nutritional requirements and the adequacy percentage according to the Food Pyramid.
• Develop educational materials with basic food groups and include them in a model diet that contains calories needed by a student.
MATERIALS AND METHODS .- An descriptive study was done over the students of the eighth and ninth years of basic education at Javeriana Sayausí College School Year 2009 – 2010, who were enrolled and was attending classes at a number of 151 adolescents, divided into 98 men and 53 women. A 24-hour recall survey and the frequency of food consumption during the week, after that, an exposition was done about basic food groups, the Food Pyramid, a balanced diet and presentation of a sample menu for adolescents according to their activities. We performed a calculation of energy intake, nutritional status and the respective classification and comparison of the percentiles established.
Results .- Of the sample taken 64.9% were male and 35.09% female. The average age for males was 13.5 ± 1.3 years and for women was 13.6 ± 0.95 years. 4 women and 6 men had short stature. 78.6% of men eat milk and other milk products a week, 21.4% don’t eat milk products. 84.9% of women eat milk products among the week and 15.1% don’t. Women eat more meat and cereal than the men and the cereal consumption of men is larger than women. 57 adolescents were below normal status according to BMI index. During meal times the carbs consumption was lower than recommended level, while fats and proteins is consuming in large amounts.

Introduction: Food security is the access to healthy and nutritious food, when
the pillars on which it is sustained fail, there is food insecurity. Maintaining food
security is the goal of all countries; however, since the end of 2019 it has become
a real challenge due to the COVID - 19 pandemic (2).
General objective: To determine food security after confinement in adults in the
urban area of the Gualaquiza canton, 2021 - 2022.
Methodology: cross-sectional analytical study that allowed determining food
security in the urban area of the Gualaquiza canton. A survey was applied to330
people with sociodemographic data, socioeconomic status and FIES scale.
The data was tabulated in SPSS and analyzed with frequencies, percentages,
chi square, prevalence ratio and 95% confidence index.
Results: The female gender predominated (65%), mestizo ethnicity (88.8%);
single (42.42%), high school graduates (43.54%) and typical C+ Average
socioeconomic stratum with 42%. 24.85% had food security after confinement
while 19.7% had moderate insecurity and 3.3% severe insecurity. Food insecurity
was associated with ethnicity (p: 0.044), education (p: 0.004), and socioeconomic
status (p: 0.000). Women were 1.17 times more likely to have it (PR: 1.17 CI:
1.01-1.35).
Conclusion: Food security was affected after confinement in the study group
since only a quarter of the population had it and women were the most affected by
food Insecurity.

Background: hypertension is a public health problem, 38 million individuals die each
year from non-communicable diseases, and 16 million die before reaching 70, in addition
to its close relationship with the lifestyles.
Objective: establish the Prevalence of cardiovascular risk in hypertensive patients over
50 years of age and its relationship with lifestyle in the outpatient clinic of the Type C
Maternal and Child Center and Emergencies IESS Cuenca, in the year 2020.
Method: a cross-sectional, quantitative analytical study was carried out with 159 people
over 50 years, using the Framingham test to determine cardiovascular risk, the
FANTASTIC survey before the signature of the informed consent. The data was
analyzed with the SPSS free version program with descriptive statistics.
Results: the mean age of cardiovascular risk was between 61 to 70 years with 40.3%,
of females with 60.4%, basic education with 67.9%, urban residence of 89.3%, with low
cardiovascular risk of 1.3%, moderate of 6.9%, high with 6.3% and very high with
85.5%, with a bad lifestyle of 13.2%, regular of 67.9%, good of 4.4% and excellent of
14.5%. A positive association was found between high cardiovascular risk and very
high with lifestyle bad.
Conclusion: a higher prevalence of women, between the range of 61 to 70 years with
the mean age of 68.9 years, of urban origin, highlighting that cardiovascular risk
increases in the population with a bad lifestyle.

Background:
Stress is one of the major health problems from workers in many fields. Nowadays, the nurse, have high stress levels, due to a lot of factors such as: social, occupational and familiar, reducing the performance of care activities to the patients even personal activities.
Goal: To implement an intervention program to reduce work stress in the nursing staff from the Cuenca’s Hospital “Vicente Corral Moscoso”
Methods and materials: A quasi-experimental studio was proposed between March 2011 and January 2012. The main group was composed by nurses and nursing assistants from Vicente Corral Moscoso Hospital.
Epi Info software was used to calculate the sample’s size (53 nurses for each group), adding 10% of loss estimate. Data were collected in previously validated forms; the information was processed in SPSS and Excel.
Results: 120 persons were taken in a random way, 60 from the experimental group and 60 from control group, in the intervention group, 84.5% were women and 3.4% men and the average age was 47 years old. In the experimental group the participants were 41.1% of Nurses and 58.8% were Nursing assistants. Before the intervention the study groups were statistically similar. The stress level in the intervention group decreased about 19.61% and making the Tpariada test, the null hypothesis was ratified with a P value of 0.005

Introduction: Child malnutrition is a preventable public health problem, which despite multiple prevention policies has not decreased. Objectives: To establish the relationship between maternal knowledge, attitudes and practices in nutrition and the nutritional status of their children in patients attending the Centro de Salud de El Valle, Cuenca-Ecuador 2023 Methods: A cross-sectional quantitative study was carried out at the El Valle Health Center, the population of this study was constituted by the binomial guardian/child between 6-23 months, with a sample of 265 infants (135 boys and 130 girls). The method used was observation, the technique used was the interview for filling out the form and taking anthropometric measurements. For the inferential analysis, 3 logistic regression models were used, with nutritional status as the dependent variable; for the predictor variable of interest, knowledge, attitudes and practices were evaluated, with a value of p<0.05 being considered significant differences. Results: When relating the attitudes and practices of the mothers, it was found that the height/age variable is significantly affected by the attitudes and feeding practices of the tutors. Regarding the attitudes in nutrition, a statistically significant positive association was found in the height-for-age indicator vs. the degree of perception of less difficulty in food diversity and frequent feeding. Conclusion: It is essential to provide adequate knowledge and education to mothers and caregivers to ensure the proper functioning of organic functions, such as the growth and development of their children.

Antecedent: the normal pregnant woman has changes in her blood pressure that do not exceed the expected limits within the physiological changes, any elevation above normal is a cause of morbidity and mortality. In practice, we use the values of the Practice Guide on Hypertensive Disorders of Pregnancy adapted from the Society of Obstetricians and Gynecologists of Canada because we do not have a blood pressure curve for our pregnant women.
Objective: to determine the variability of blood pressure in a population of normal pregnant women in the city of Cuenca.
Methodology: descriptive, longitudinal study; Carried out in the external consultation of 8 centers of the National Health System of Cuenca. The pressure was measured from the 12th to the 40th week. A blood pressure curve was drawn: systolic, diastolic and mean.
Results: 338 pregnant women were studied in which the systolic blood pressure was 96.24 ± 8.36 Torr units (range 70 - 122), diastolic 60.18 ± 7.35 (range 40 - 81) and mean 72.12 ± 6.92 (range 50 - 93). The systolic blood pressure curve shows a rise up to the 38a week, while the diastolic blood pressure curve shows an irregular behavior that ends up ascent until the end of the 40a week and the mean arterial pressure curve shows a more noticeable behavior until week 38a.
Conclusion: The systolic, diastolic and mean blood pressure averages show a progressive increase with respect to the end of the pregnancy that agrees with what the theoretical basis indicates, but, limits lower than those established as reference standard

Background: the well-being of mothers during the birth process is closely related to the
satisfaction of their needs and the treatment received, a situation that has been threatened
worldwide.
Objective: to determine the level of maternal well-being during childbirth at the Mariano Estrella
Cuenca Type-B Health Center. 2022.
Methods: a descriptive quantitative study, we worked with a finite universe during the months of
August to October 2022, using the scale of maternal well-being in childbirth as an instrument
(EBMSP2) developed by Uribe et al, with a Cronbach's alpha of 0.937.
The data were processed with the SPSS version 25 program, presented in tables of frequencies
and percentages.
Results: 83% of the mothers have an optimal level of well-being. 100% reported having obtained
a comfortable physical environment in their care. 45% of the mothers did not have a choice of
accompaniment in the three moments before, during and after the delivery, among the noncompliance criteria was misinformation (17%) and arriving without a companion (14%).
Conclusions: despite having mostly obtained an optimal level of well-being, there are low but
striking percentages regarding: maternal follow-up, prenatal and intranatal prophylaxis, self-care
and comfort related to the choice of delivery position, as well as complications. presented

Objective: determine the prevalence of nasopharyngeal carriers of Staphylococcus aureus, antimicrobial susceptibility pattern and associated factors in elderly caregivers in the geriatric centers of district 6 in the city of Cuenca during 2016.
Material and methods: A cross-sectional study was conducted. The sample size was randomly calculated with 206 participants. The investigator performed the sampling of nasal passages, cultivating on blood agar and mannitol. We consulted on the associated factors, after the data collection were introduced on a basis in SPSS version 15.
Results: in the study 89.9% were women. The age median was 35 years. The prevalence of nasal carriers of Staphylococcus aureus was 16% (95% CI: 11.2-21,8). The same had a sensitivity of 100% to vancomycin, cefuroxime and novobiocin. It was associated with diagnosis of chronic pathology (RP 2,813 95% CI: 1,467-5,396 p <0.001); use of daily medication (RP 2,944 95% CI: 1,576-5,498 p <0.001) and hospital visit (RP 2,377 95% CI: 1,126-5,017 p 0.016).
Conclusions: The prevalence found differs from other studies reviewed and was statistically associated with diagnosis of chronic pathology, use of daily medication and hospital visit, the bacteria was sensitivity a vancomycin, cefuroxime and novobiocin similar to studies in Latin America and different from European countries

This research aims to identify the perceptions of women survivors of violence, about satisfiers that contribute to their recovery process within the eco-friendly system, Casa María Amor 2014.
This is a qualitative study that has been done through nine semi-structured interviews or focus group, two days of participant observation and documents review; to identify the perceptions of women which are survivors of violence about the refugee and support in Casa María Amor and also their perceptions about the synergistic satisfiers: self-esteem, autonomy, solidarity and critical consciousness developed in an eco-friendly system and to contribute to the process of recovery. The ecosystemic approach including a gender perspective and interdisciplinary as well as theories of ecofeminism and Necesidades Humanas Esenciales were a guide for this study.
As a result of the investigation, it is found that perceptions of women survivors of violence in Casa María Amor realize that the refugee and support received promote the development of synergistic satisfiers: self-esteem, critical consciousness, autonomy and solidarity; thus contributing to their recovery process towards meeting their basic human needs.
The ecological model and eco-friendly system of Casa María Amor in Cuenca, it makes of this a unique experience to be systematized, it can also work as a model for other shelters with similar objectives, and thus amplify its contribution to women seeking alternatives to support gender violence

Background: deaths due to violence are a worldwide phenomenon, which have a negative impact not only on general mortality figures, but also greatly increase the costs of health, social and/or judicial services.
General objective: to analyze violent deaths in the canton of La Troncal in the province of Cañar, during the year 2021.
Methodology: descriptive study. 90 autopsies performed during the study period were analyzed. The information was recorded on a data collection form. Descriptive statistics were used, the information was presented in personalized tables and graphs. All bioethical procedures were applied to ensure data confidentiality.
Results: of the 90 medical-legal analyses, 86.67% of the deceased were adults, 77.6% male, 92.22% Ecuadorian; 76.6% were single, 95.56% mestizos and 30% day laborers. The prevalence of violent deaths was 80%. According to the manner of death, 55.56% were homicidal acts, followed by 43.06% traffic accidents and 1.39% attributed to suicide. Firearms were involved in 90% of homicides; Regarding traffic accidents, 51.61% were due to a motorcycle or vehicle collision, followed by 32.26% due to crashes. As for the only case of suicide, this event was due to intoxication by organophosphate.
Conclusions: a high prevalence of violent deaths was evidenced in the canton of La Troncal, with percentage values higher than the consulted bibliography.

Background: The postpartum period in women is a stage where factors that can influence sexual activity occur. Despite being a prevalent problem, adequate importance is not given and female sexuality is considered an exclusively reproductive function.
Objectives: To identify the prevalence and factors that condition and/or determine whether women can return to sexual activity in the first 6 months after childbirth during the period March-August 2022.
Methods: cross-sectional quantitative and analytical study, carried out in the gynecology area of the Municipal Hospital of Cuenca in a sample of 180 puerperal women. A form was created with sociodemographic, obstetric, biological and psychological data from the EVAS-M. Statistical analysis was performed with SPSS v.24 software; results were presented through frequencies, percentages, averages and standard deviation; the association of factors was analyzed using the chi- square test, with significance les than 0.05.
Results: the average age was 29.24±6.90 years; 49.4% married; 51.7% had a secondary education level; 85.0% lived in urban areas; 60.6% had a cesarean delivery; 69.4% were indifferent to sexual satisfaction; 12.8% have years of dissatisfaction; 44.4% returned to sexual activity for their partner and 50.6% returned to sexual activity after 40 days.
Conclusions: the prevalence of the return to sexual activity of postpartum women in outpatient consultation at the Municipal Hospital of Cuenca was high and was associated with the level of education, time and cause of return, vaginal delivery, postpartum time, tear and episiotomy.

Introduction: malnutrition in the elderly is a major concern worldwide. It is a public health problem and it is associated with a number of factors, some of them are modifiable.
Objective: to determine the prevalence and factors associated with malnutrition in older adults in Gualaceo town, 2016.
Methodology: an analytical cross-sectional study. Sampling was probabilistic, stratified and randomized, which assessed 250 elderly. Nutritional status was assessed using the Mini Nutritional Assessment (MNA). Data were tabulated in SPSS version 20.0. Characterization of the population was performed through descriptive statistics and central trend measures, statistical association was measured with prevalence ratio (RP), 95% confidence interval (IC) and values of p <0.05 were considered statistically significant. In addition, a multivariate analysis was performed.
Results: the prevalence of malnutrition was 20,4%. The average age 76,31 (± 8,19). The associated factors were: depression RP 4,90 (95% CI: 3,08 – 7,79, p< 0,000); sedentary lifestyle RP 4,52 (95% CI: 2,81 – 7,28, p <0,000); low socioeconomic level RP 4,04 (95% CI: 1,51 – 10,7, p<0,001); anemia for both men and women RP 2,43 (95% CI: 1,23 – 4,77, p<0,018) and RP 4,02 (95% CI: 2,17 – 7,63, p<0,000) respectively and hypoalbuminemia RP 2,87 (95% CI: 1,08 – 4,58, p<, 0,000). Multivariate analysis corroborated this association except for hypoalbuminemia.
Conclusions: the prevalence of malnutrition was 20,4% and it is associated to: depression, sedentary lifestyle, low socioeconomic status, anemia and hypoalbuminemia

Background: Globalization, urbanization, inequalities, are factors that are producing negative
changes in the food situation of children, as well as the beliefs and food practices of parents
and/or caregivers of children in a state of dependency and/or with disabilities, which will
influence their normal growth and development. Objective: To identify the dietary beliefs and
practices of parents and/or caregivers of children with intellectual disabilities who attend the
Instituto Piloto de Integración del Azuay del Cantón Cuenca 2022. Methods: A descriptive
qualitative study with a phenomenological approach was conducted. The sample, by snowball,
was constituted by parents and/or caregivers of children attending the Instituto Piloto de
Integración del Azuay. Direct surveys were conducted and transcribed in Word format. With
the help of Atlas Ti software, codes and quotations were made for the subsequent
interpretation and writing of results. Results: Participants' beliefs and knowledge identified in
the discourses determined practices around balanced eating, appropriate food storage, and
appropriate personal and utensil hygiene. The functional needs of chewing and swallowing of
children and adolescents with intellectual disabilities were considered. Heterogeneity was
observed in the subjects' participation in food preparation, food assistance, feeding schedule
and use of feeding instruments. Conclusions: Appropriate feeding and hygiene behaviors were
identified and practices coincided with beliefs and knowledge that were transmitted from
previous generations.

Identify the prevalence of low school yield and associate factors in adolescent student’s whit emigrant parents of the urban schools of Gualaceo during the year 2004-2005. Material and methods: Traverse study carried out by a survey to students of the urban schools of Gualaceo, Azuay, Ecuador in ages between 10 and 19 years. A random sample was generated, stratified according to sex and school type. Results: They were carried out 221 surveys which 32,6% has low yield of these 58,3% they correspond to: half adolescence; 66,7% masculine sex; 80,6% doesn't work. The schools: 95,8% are public; 76,4 mixed%. Family characteristics: 66,7% incomplete families; 54,2% for emigration; 41,7% father contributes economically. Of the parents: 82,4% mothers, 95,5% parents, <34 years; 61,1% mothers, 54,2% parents with primary studies; 59,7% mothers carries out domestic chores; 32% parents is employees. Emigration: 47,3% emigrates; 27,8% the father; 58,3% to USA. Emotional dysfunctions: 23,6% has anxiety; 34,7% depression; consumption of alcohol: RP: 1,7434; IC95%: 1,1132-2,7304; VP: 0,0051; to smoke: RP: 2,1991; IC95%: 1,4939-3,2374; VP: 0,00002. School absenteeism: if 1,4%. Educative project is being elaborated. Conclusions: The low yield is independent of the emigration of its parents, it is presented in: the half adolescence, masculine sex, public school.

Background: the elderly are at risk of presenting pathologies such as
malnutrition and depression. Its early detection can prevent health
complications in this population group.
Objective: to determine the prevalence of the risk of malnutrition and its

Methods: cross-sectional analytical study, which included 350 older adults
from the urban area of Cuenca. Nutritional status was assessed using the Mini
Nutritional Assessment and the presence of depression using the Geriatric
Depression Scale. Data were tabulated in the free SPSS Version 21program.
For the analysis, descriptive statistics were used such as frequency,
percentage, measures of central tendency, measures of dispersion (standard
deviation) and for association: prevalence ratio (PR), with Confidence interval
(CI) of 95%, and Chi Square Pearson's for statistical significance, with its value
p < 0.05.
Results: the prevalence of malnutrition risk was 36%. The prevalence of
malnutrition was 8.86%. The most prevalent group was 65 to 74 years old
(54.86%). The risk of malnutrition and malnutrition were significantly
associated with age, marital status, and educational level (p < 0.001). In
addition, there is a strong association between the risk of malnutrition and
depression, the PR of 7.293 (95% CI 4.678 - 11.369, p <0.001).
Conclusions: the prevalence of malnutrition risk was 36% and was
associated with age, marital status, educational level and depression.

INTRODUCTION: 84% of patients have postoperative pain, there is not a consensus control, and the preemptive analgesia (PA) is an interesting alternative.
OBJECTIVE: To determine the analgesic efficacy of levobupivacaine – ketoprofen combination administered before the nociceptive stimulus (preoperative) compared with post laparoscopic cholecystectomy administration (after surgery), Homer Castanier Hospital.
METHODS: A randomized clinical trial single-blind was conducted, 100 patients into two groups are divided, 50 received preoperative ketoprofen – levobupivacaine combination (experimental group) and 50 received the same combination in the postoperative period (control group), to measure the intervention effectiveness the pain intensity was assessed -using the Visual Analogue Scale (VAS)- and additional analgesic requirements postoperatively. The analysis was performed using chi-square test and Student t test. The P value p<0.05 was considered significant.
RESULTS: Both groups were comparable (p>0.05). Experimental group had a lower postoperative pain intensity than control group at first 24 hours (p <0.05), after 24 hours the difference was not significant (p=0.063); The PA provided a lower risk of postoperative pain with RR=0,313 and ITT=2.27. The average supplemental analgesics doses are used was significantly higher in the control group 2.3 ± 1.03 than the experimental group 1.2 ± 0.72 (p <0.05), but the PA did not eliminate need for additional analgesia.
CONCLUSIONS: Ketoprofen + levobupivacaine combination was effective, pain intensity decreased and the additional analgesics doses postoperatively were lower too, but did not eliminate the need for additional analgesia.

Background: Malnutrition in chronic kidney patients is frequent, recent studies
agree that quality of life decreases and morbidity increases. The evaluation of
nutritional status analyzes the individual from biochemical, dietary,
anthropometric and clinical aspects, in order to carry out timely nutritional
intervention, minimizing the risk of malnutrition.
Objective: To determine the evolution of the nutritional status in patients who
attended a Guayaquil Hemodialysis Center from January 2017 to February 2020.
Methods: Descriptive, correlational study, collected information recorded in 155
medical records of patients over 20 years of age with chronic kidney disease who
attended the Guayaquil Hemodialysis Center between January 2017 and February
2020. Data collected from a software for medical records, which records
anthropometry, biochemistry, dietetics and clinical. It excluded cancer patients,
pregnant women, with incomplete history. The Pearson correlation coefficient test
was used to measure the relationship between nutritional status and chronic kidney
disease in advanced stages with hemodialysis.
Results: Statistical analysis determined the evolution of nutritional status for 3
years, relating variables nutritional status and chronic kidney disease in advanced
stages with hemodialysis, associated with age, sex, residence and
socioeconomic situation, comparing initial nutritional status, which on average was
25.63 kg. /m2, with a final average of 24.95 kg/m2.
Conclusions: It was found that kidney disease occurs regardless of age, with a
predominant male sex, indifferent socioeconomic situation. There was no positive
relationship between biochemistry and anthropometry. Four of six observed
parameters presented improvement, therefore, there was a positive evolution of
health within their condition.

Background: Health professionals are aware of the benefits of a correct diet and
nutrition to maintain an optimal state of health, avoid diseases and complications,
however, in them there is an inadequate food consumption.
Objective: To analyze the relationship between diet quality, physical activity and
nutritional status in the health personnel of the Seguro Social Campesino (IESS)
of Azuay, November 2021- March 2022.
Methods: A cross-sectional analytical study was conducted in 80 health
professionals of the Dispensaries of the Seguro Social Campesino (IESS) of
Azuay, by non-probabilistic random sampling, using food consumption frequency
surveys, 24h recall, anthropometric data (body mass index, waist-height index),
international physical activity questionnaire. The data were entered into the SPSS
program, applying descriptive statistics such as frequency, percentage, measure
of central tendency (average) and dispersion (standard deviation), for measuring
association, prevalence ratio (PR) was used, with 95% CI, Pearson's chi-square
(p) with statistical significance < 0.05, indicated in graphs and tables.
Results: 43.75% of the population was overweight,16.25% grade I obesity,
52.50% a low level of physical activity, 53.75% excessive carbohydrate
consumption, 42.50% excessive calorie consumption, 51.25% need changes in
their diet.
Conclusions: A diet quality with excessive consumption of carbohydrates,
calories and low physical activity is associated with overweight/obesity and high
waist to height ratio.

BACKGROUND: The limitations of basic activities of daily living (Basic ADLs), and their effects on health in the elderly, are a public health problem.
Objective: To determine the prevalence and perceptions of limitations in basic activities of daily living and associated factors in older adults living in the geriatric centers of zone 6 of Ecuador 2016.
Materials and Methods: A quantitative and qualitative cross-sectional and phenomenological study in 158 older adults. Katz test, International Physical Activity Questionnaire, Folstein Minimental State Examination, Yesavage Geriatric Depression Scale, and Comorbidity Index Charlson were used. Quantitative variables were analyzed using descriptive and bivariate statistics. In the qualitative phase, two interviews and three discussion groups were carried out. The results were analyzed using the software SPSS version 15 and Atlas-ti.
Results: The prevalence of functional limitation in the studied population was 54%, being higher in women. The risk factors associated were: depression (RP 1,691, CI (1,805-7,857) and osteoarthritis (RP 1,805, CI (1,760-6,630), with a p <0.0001.) Self-evaluation (RP 0.840, CI 0.125-0.473), Physical activity (RP 0.562; IC 1. 0.085-0.471), were protective factors with a p <0.0001.
The perceptions in the elderly were related to the losses of the physical conditions, loved ones and socio-economic position.
Conclusions: The prevalence of functional limitation in the elderly is high. The associated risk factors were depression and osteoarthritis.

Background: Diabetic foot have devastating complications, including amputations, poor quality of life, and life-threatening infections. They are costly and consume health care resources. These consequences have serious clinical and public health implications.
Objective: To determine the frequency of diabetic foot disease and its relationship with associated factors in patients with type 2 diabetes mellitus treated in the health facilities that make up District 01D04.
Material and Methods: A retrospective cross-sectional analytical study was carried out with 147 medical records that met the inclusion criteria, the data obtained were entered and analyzed in the SPSS 20 program.
Results: prevalence was 4. 8%, 71.4% were men, the most affected population was >60 years old with 85.7%, 85.7% had a disease evolution of more than 10 years, 71.4% had comorbidities, 28.6% had hypertension, dyslipidemia, nephropathy, respectively (2 patients for each comorbidity), none had neuropathy or a history of amputation, 57.1% had a BMI below 29.9, 100%, 85.7% had an HbA1C of >60 years old with an HbA1C of >60 years old, 85.7% had an HbA1C of >60 years old with an HbA1C of >60 years old with an HbA1C of >60 years old, 7% had HbA1C above 7%, 71.4% had cholesterol and triglycerides in normal values, and 71.5% used insulin, there was a statistically significant association in time of disease evolution >10years, presence of comorbidities, having HT, nephropathy, retinopathy, neuropathy, history of amputation, BMI >30, glucose >126mg/dl, HbA1C >7%, cholesterol >200mg/dl, with P-value <0.005.
Conclusions: It is more frequent the development of diabetic foot in patients older than 60 years, men, with low schooling, who resided in urban area, who used insulin.

Background: Community-acquired pneumonia is one of the pathologies with the highest morbidity and mortality in older adult patients globally, with a significant percentage requiring hospitalization.
Objective: To establish the prevalence and identify the factors associated with the hospitalization of elderly patients with community-acquired pneumonia treated at José Carrasco Arteaga Specialty Hospital. Azuay-Cuenca, 2019.
Materials and method: a cross-sectional analytical study was carried out, with a sample of 238 medical records of older adult patients, later the respective data were collected using a form, which were exported to the SPSS version 25 program and a descriptive and analytical analysis was carried out.
Results: a prevalence of CAP of 21% was found, CI 95%=15.6 - 26.4. The most prevalent months were January, May, August and September. The average age was 81 years, predominantly female 60%, the hospital stay had an average of 10-12 days, the specific mortality rate was 16%. Statistical significance was found in the clinical, laboratory and imaging characteristics with a P value < 0.05. The most frequent comorbidities were chronic obstructive pulmonary disease (27.6%), cerebrovascular accident (26.9%), heart failure (25%). Living in the care of a caregiver was a protective factor for CAP, while the predisposing factors had statistical significance with a p<0.05: recurrence and the first infection of CAP, previous care, living with the family, in the nursing home and alone.
Conclusions: BIRTH in the elderly is a health problem that is maintained over time, with high hospitalization rates and therefore greater use of health resources.

Background: Alcohol consumption in adolescence is frequent, studies reveal that consumption is associated with some social determinants.
Objective: to identify the prevalence of alcohol consumption among adolescents at Daniel Córdova Toral School in the city of Cuenca-Ecuador and its relationship with social determinants.
Materials and methods: cross-sectional quantitative study, 218 adolescents. A form was used for data on demographic variables, family structure and migration. For the prevalence of alcohol consumption, the DSM5 diagnostic criteria were used, for the FF-SIL family test and for the FANTASTIC lifestyle test. To determine the level of association between social determinants and alcohol consumption, PR was obtained with its 95% CI, and to determine statistical significance, p value was obtained by Chi square, being statistically significant when p was less than 0.05.
Results: The prevalence of alcohol consumption was 42.2%; They come from a nuclear household 56%, 55% have a migrant family, have a good lifestyle 39% and 50% belong to moderately functional families. Having a migrant family member, having a bad lifestyle, belonging to a reconstituted and dysfunctional family are risk factors for alcohol consumption.
Conclusion: Alcohol consumption is higher than reports of other studies conducted in the city of Cuenca and is associated with the social determinants studied

Background: The suicidal ideas are the essential component of suicidal behavior,
in which a person voluntarily thinks about their own death, these ideas are present
in almost all age groups, with a significant increase in the adolescent population,
becoming a problem of public health.
General objective: to determine the risk of suicide in adolescents aged 13 to 18
years of the Unidad Educativa 26 de febrero, of the city of Paute, during the month
of march, in the year 2022.
Materials and methods: it is a quantitative, observational, prospective crosssectional study; with a sample of 214 students from the Unidad Educativa 26 de
febrero in the city of Paute, during the month of march, between the ages of 13 and
18 of both sexes, using the ISO 30 test (suicide risk inventory) as a study
instrument.
Results: the suicidal risk was high 50.5% and moderate 44.9%, the sub-scale shows
87.4% hopelessness, 50.5% loneliness and depression, 52.8% low self-esteem,
75.7% difficulty coping with emotions and with 60.3% suicidal ideation.
Conclusions: there is a high risk and moderate of committing suicide in adolescents
aged 13 to 18 years, being higher after 16 years and in females.

Background: When analyzing the family, it is not a question of adopting the concept of Western culture, which indicates a group formed by husband, wife and children1. The family is seen as a system, parents and children are subsystems that interact2. Many times this part is invisible. Different disciplines have studied the family but the results they present are fragmented3. It is not analyzed in depth the family dynamics, in its structure, its typology, hierarchy, roles, holons, triangulations, relationships, repetitive patterns, functionality. People with obesity are one of the populations with physical, social and psychological problems that force the family system to adjust to new circumstances.
Objective: To describe the perception of people with obesity about their family dynamics. Methodology: Qualitative phenomenological study, the answers were categorized in family stereotype, life events, difficulty to the displacement, gender role, patriarchal authority, false support. An in-depth interview was applied to the index person (with obesity), through the application of familiogram and FF-SIL test. The sample was saturated with 23 interviews.
Results: We found that the family structure of people with obesity is of different type, presenting more nuclear families. Family dysfunction excelled in people with severe obesity. The interviewees' discourse allowed us to identify consolidated social representations.
Conclusions: Obesity is a repetitive pattern, with defined roles, with a gender focus, which makes it difficult to lose weight because family members do not provide real support

Tooth decay has become a permanent problem for public health worldwide, destroys the tooth as it is not treated tissues, this we know from the moment we walked into the university classroom to exercise our professional life and to check this study was to dental students as part of the university population.
Objective. Determine the status of oral health as prevalence of caries, oral hygiene and more frequent dental students at the University of Cuenca oral pathologies.
Methods. 295 students from the Faculty of Dentistry at the University of Cuenca (FOUC), was enrolled in the period studied 2013 - 2014 and a sample of 154 students between 17 and 26 years of first to fourth year of study was selected.
Results. Caries prevalence of 89% DMF index was presented - D average was 0.6 ± 0.7 between a minimum of 0 and a maximum of 2.0 gingivitis also appeared in an average of 75.9%.
Conclusions. There is a prevalence rate similar to other countries worldwide otherwise of severity of caries is better in the group of students (CPO D) There is a lot of control on the part of students and that is not equal severity prevalence seen in this way oral care not prevent dental caries but if its severity, but cannot say the same for gingival health by the presence of gingivitis which is present in a majority group of the sample.

Background: Obesity and overweight represent a problem worldwide. This phenomenon has been linked to several factors, including emotional factors. Objective: To establish the relationship between nutritional status and emotional eating in adults belonging to the El Valle parish. Materials and methods: Observational, analytical cross-sectional study. The sample was 414 adults applying the proportion formula. The information was analyzed in the free software PSPP 1.2.0. To measure the association of the factors, chi square, Fisher's test and the prevalence ratio were used with a 95% confidence interval and a statistical significance of p < 0.05. The study was approved by the Human Research Ethics Committee of the University of Cuenca. Results: the female population participated in a higher proportion (63.3%), with ages between 20 - 29 years. Regarding the nutritional status, overweight and obesity are present in 56.6% of the population. 51.9% and 71.3% of men and women respectively are emotional eaters. A statistically significant association between nutritional status and emotional eating is estimated. In addition, an association was found with: hours of sleep (PR 1.41 IC 1.05 - 1.88), physical activity (PR 1.61 IC 1.22 - 2.11) and depression (PR 1.48 IC 1 .13 – 1.95); all statistically significant (p < 0.05). Conclusions: More than half of the participants are overweight or obese, with a high frequency of emotional eating; with a statistically significant association between these two variables. The factors associated with nutritional status were: presence of depression, fewer hours of sleep and low level of physical activity.

Background: mechanical asphyxia represents one of the most prevalent causes
of violent death worldwide; its etiology is as diverse as its types. Findings found at
autopsy range from general to more specific, related to the type of asphyxiation
that caused death.
Objective: establish the frequency and characterization of deaths from mechanical
asphyxia registered by the National Forensic Science Service of Loja, during the
period from January 2017 to December 2020.
Methodology: quantitative, observational, descriptive, cross-sectional approach.
Data collected from existing medical-legal autopsy protocols at the Forensic
Science Center, inclusion criteria within the period from 2017 to 2020, being the
sample the whole universe. The information is collected through a form prepared
by the author, including variables of interest. Database elaborated in Excel,
statistical processing performed in SPSS, using statistical mode.
Results: the highest number of deaths from mechanical asphyxia occurred in the
male population, age group from 25 to 65 years old, more frequently in singles, the
most common cause of mechanical asphyxia is hanging, followed by suffocation;
the most frequent etiology was suicidal, followed by accidental.
Conclusions: the male, single, and adult population, is mostly exposed to this type
of event, mainly of a suicidal nature. General and guiding signs are not
pathognomonic, but they represent an important tool for proper diagnosis within a
legal investigation.

Social participation (SP) has a great development in Ecuador, social organizations such as indigenous people, housewives, workers have found in the organization a mechanism to be heard and to make their needs known. The importance of SP to meet health-related objectives is well recognized. Currently, there are scientific documents and laws that protect and promulgate this right, but SP is established according to political will, and is mediated by multiple factors including knowledge of collective rights, the training of professionals and the interests of the macrostructure. Objective: to describe the practices of social participation in leaders of the Gualalcay community, according to the Elizabeth-Anderson model. Methodology: qualitative - ethnographic, it was carried out through in-depth interviews with community leaders, which were recorded and transcribed for analysis, based on Elizabeth-Anderson's model. Results: SP distributes power and gives voice to the community, the model allowed systematizing the practices of the leaders, identifying that the actions improve the perception of community health and revealing the need for empowered communities, trained professionals, discarding hegemonic paradigms and recognizing community wisdom. Conclusions: SP can be adapted to the reality of each society, but it is important that civil society and health professionals recognize their roles in this coexistence and development of health. Elizabeth Anderson's model allowed the valuation of the community in reference to SP.

Background:In the city of Cuenca there have been no studies to indicate awareness and public participation in relation to air pollution, or the possibility of integration with institutional levels.
Objectives: To identify concepts, attitudes and practices of the population in relation to air pollution and the impact in health, to establish management roles of institutions and organizations involved.
Method:Descriptive Study CAP by opinion questionnaire. Study population, persons over 15 years, of Cuenca city. Sample 500 people. Information collecting:by interview of open questions, but with pre-coded response registration. The results were analyzed in SPSS 15.00 program version evaluation.
Results:
Concepts and knowledge 44% having a high level, 19% have very high levels. Is minimal (6.2%) the low level, attitudes and motivations: have a favorable attitude (48.8%), very favorable (31.2%), practices: most of the population (56.6%) have less adequate practices and 28.6% have inadequate practices, appropriate practices are minority. The correlation of concepts, attitudes and practices of affiliation and residence variables are differences, having groups with higher levels of concepts have a lower level of practice and vice versa. We analyzed the characteristics of institutions, their roles and their willingness to promote health.
Conclusions: Concepts and attitudes appropriate, less adequate practices, no concordance between concepts and practices in some groups.

OBJETIVO
Desarrollar, aplicar y evaluar un programa educativo virtual dirigido a médicos rurales de las provincias del Azuay, Cañar, Morona Santiago y Loja para la correcta prescripción de antibióticos en faringitis, infección del tracto urinario, diarrea infecciosa y piodermias.
METODOLOGIA
Estudio experimental aleatorizado doble ciego que contó con la participación de 102 médicos rurales de las provincias mencionadas durante el periodo febrero- octubre de 2011. Mediante plataforma virtual MOODLE se aplicó un curso sobre prescripción de antibióticos basado en las guías OPS, Sandford y del MSP a 53 médicos del grupo intervención y en otra plataforma un curso sobre “Análisis crítico de promoción farmacéutica” a 49 médicos del grupo control. Se evaluaron mediante aplicación de cuestionario antes y después los conocimientos adquiridos y la calidad de recetas antes y después del curso.
RESULTADOS
El curso virtual mejoró la media del puntaje de cuestionarios de 7,5/10 a 8,07/10, (p= 0,037) en el grupo intervención, así como el puntaje de las recetas en todos los diagnósticos. El riesgo relativo de la intervención sobre la presencia de mala prescripción fue de 0,53 (p= 0,0) y sobre la mala calificación en cuestionarios de 0,78 (p= 0,0).
CONCLUSIONES
La aplicación del programa educativo virtual por la plataforma MOODLE en médicos rurales, mejoró los conocimientos y la calidad de prescripción en consulta externa para faringitis, infección del tracto urinario, diarrea infecciosa y piodermias.

The appearance of acne after 24 years in women is associated with hormonal and metabolic factors, although cardiovascular risk has been studied at later ages, recent studies imply a relationship between acne and association with insulin resistance and dyslipidemia in The young woman, to identify the presence of cardiovascular risk factors in this group of young patients, would help to take preventative mediated in an earlier form.
Methodology:
An analytical cross-sectional study, which included 232 women older than 24 years of age who were seen at the Vicente Corral Moscoso Hospital from July to December 2016 and who met the inclusion criteria, were evaluated in blood: glucose, insulin, HDL, LDL, triglycerides , As well as blood pressure, abdominal waist and body mass index
Results:
The prevalence of Acne was 39.2%, the mean age was 31.6 years; it was more frequent in women with third level of education (45.7%) the Insulin Resistance RP of 2,275 (CI 1.7 -3) and abdominal waist RP 0.7 (CI 0.5-0.9) were the risk factors that presented relationship.
Conclusions:
Insulin resistance was considered as a risk factor for acne while the abdominal waist was considered as a protective factor

Background: Nursing due to the nature of their profession is exposed to physical and psychological risk factors, in addition to the external influence of family, economic, health and personal factors, their Quality of Work Life can be affected, influencing in this way such climate and organizational efficiency, as well as the treatment provided to the patient.
Objective: to determine the level of perception of the quality of working life and the associated factors in the nursing staff of the José Carrasco Arteaga hospital.
Materials and methods: cross-sectional observational study using the CVT-GOHISALO questionnaire for the evaluation of quality of work life, x2 was applied to seek association with personal, family, health and economic factors.
Results: Of the participants, 95.2% were women, the average age was 34.8 years, of the study factors found to be associated with CVL sex, to live in dysfunctional families, to have children under two years, Time to the domestic role and the socioeconomic level.
Conclusions: nurses have an average CVL, however, greater attention should be given because 41.6% of the patients evaluated as low. We determined the factors that are associated with low CVL

Sexuality has been defined as the way in which people experience and express the instincts and feelings that constitute physical attraction to others. A good part of human sexuality is defined during adolescence. Objective: to describe the sociodemographic factors and their relationship with knowledge and attitudes about sexuality in high school students of the Bilingual Educational Units of the Cañar Canton, 2022 to 2023. Methodology: analytical, cross-sectional study, carried out with a universe of 167 adolescents, belonging to zone 6 of Education. Sociodemographic factors were collected using a form prepared by the author. The knowledge was obtained by applying the test designed by Navarro, M, and López, A, validated through expert judgment and with an Alpha Cronbach reliability index of 0.65. Attitudes were explored using the ATSS test modified by López, F. The data were processed in SPSS and the results are presented in simple and contingency tables. Results: 6 out of 10 and 3 out of 10 students showed they had a high and medium level of knowledge about sexuality; respectively, the conservative attitude turned out to be the most frequent with 47.3%, manifesting itself especially in indigenous, Catholic adolescents, in their first and second year of high school, with age ranges of 14 to 16 years. Conclusions: there is a significant statistical association between the level of knowledge about sexuality and ethnic status. Regarding attitudes and their relationship with sociodemographic variables, no significant statistical associations were found.

Background: traffic accidents represent a global health problem, among the multiple means of transport involved, motorcycles are widely used due to their versatility, mobility, economic cost and easy driving, however, it represents an imminent vital risk to use it inadvertently. an inappropriate way.
Objective: to determine the reason and proportion of morbidity and mortality in motorcycle traffic accidents registered in the Attorney General of the State of Cañar, years 2019-2020, pre-pandemic context and COVID-19 pandemic.
Methodology: analytical, cross-sectional study. 127 medical-legal reports of motorcycle traffic accidents that met the inclusion criteria were reviewed. Descriptive statistics were applied, comparing various medical-legal aspects of morbidity and mortality before and after the pandemic; In addition, a bivariate analysis was performed.
Results: the morbidity of motorcycle traffic accidents was 52% in 2019 and 21.3% in 2020; In turn, mortality during the pre-pandemic period was 16.5% and in 2020 it was 10.2%. The ages between 19 to 29 years, males, with primary education and single were the groups where the highest morbidity and mortality occurred due to these traffic accidents. Lower limb fractures were the most common injuries with 28% and head trauma the most frequent cause of death in 67.6%. The mechanism of crash accidents and night hours were the most representative events of motorcycle incidents.
Conclusions: the frequency of morbidity and mortality due to motorcycle accidents in Cañar is very high, data that coincide with the bibliography consulted.

Objective: to determine the prevalence of surgical site infection, and its relationship with associated factors in patients with abdominal surgery that were assisted by the trauma and emergency service at Vicente Corral Moscoso Hospital in 2016.
Methods and Materials: we carried out a cross-sectional study with 206 patients that met the inclusion criteria, and signed the consent. They were observed since they were admitted until they developed an infection, or thirty days after the surgery. Variables such as age, gender, wound contamination, overweight, obesity, cigarette smoking, preoperative stay, comorbidities, surgical time, steroids or immunosuppressive, hyperglycemia, transfusions, using drainage, ICU stay, antibiotic prophylaxis, prior antibiotic, antibiotic treatment were studied. The data was entered in a database to then carry out a bivariate and multivariate analysis.
Results: the prevalence was of 17.48 %, finding significant statistic relevance in age (above 65 years of age), length of the surgery (longer than 120 minutes), preoperative stay (longer than 24 hours), hyperglycemia (above 180 mg/dL) ASA III and IV, contaminated and dirty wound, transfusions, using drainage, ICU stay, the non-use of antibiotic prophylaxis, open approach, using prior antibiotic and BMI over 35. Statistic relevance was not found on the following variables: cigarette smoking, taking immunosuppressants, antibiotic prophylaxis longer than 60 minutes and use of antibiotic treatment.
Conclusion: surgical site infection is higher to those reported, and is related to various risk factors that correspond to both, the patient, and the surgery itself

Introduction: in low-income countries, the consumption of ultra-processed foods
(UPF) has increased, in the same way, university students have an unhealthy diet
and these foods are proving to be an attractive option for this population. In the long
term, this can trigger health risks caused by various factors.
Objectives: to determine the prevalence of ultra-processed food consumption and
its relationship with lifestyle in university students in the city of Cuenca, 2021-2022.
Methods: Cross-sectional analytical study on a sample of 381 university students
from Cuenca, with prior informed consent. The ENSANUT 2012 processed food
consumption questionnaire was acquired according to the NOVA list, Ecuador. For
the lifestyle of university students, the CEVJU-R questionnaire was applied. The
analysis of the information was carried out in the SPSS V-21 software, for the
sociodemographic variables, frequency and percentage tables were obtained, for
the dependent and independent variables, association measures were taken,
prevalence ratio with 95% CI and X2
with p value. < 0.05 for statistical significance.
Results: The prevalence of UPF consumption was 99.7%. The exercise and
physical activity dimension was associated with gender. The dimension of leisure
time with the consumption of fats, consumption of sugars and derivatives,
consumption of alcoholic beverages, and the dimension of the perceived emotional
state with the consumption of sugars and derivatives.
Conclusion: there is a high prevalence of UPF consumption in the study population
and this was related to the lifestyle of university students.

Asthma is a more frequent chronic respiratory disease in childhood that varies
according to the region and characteristics of the population.
Objective: To identify the sociodemographic determinants and the quality of life
of children aged 7-12 with asthma attacks attended in emergencies at the Vicente
Corral Moscoso, José Carrasco Arteaga and the IESS Maternal and Child
Centers, Cuenca.
Method: A descriptive analysis of the sociodemographic data and the quality of
life of the children attended during May 2019 to February 2020. Frequency tables
were made for the qualitative variables and for the continuous variables means
and standard deviation were obtained. The quality of life was determined
according to the total score, Good (> 115), Fair (70-115), Bad (<70).
Results: A total of 56 children, 44% between 7-8 years of age, 53.6% female and
62.5% from urban areas, with a monthly family income between 397 and 999
dollars. In quality of life, 55% obtained a score between 70-115; limitation of
activities obtained a mean of 3.5 ± 1.21, asthma symptoms 4.1 ± 1.45 and
emotions 4.2 ± 1.43.
Conclusion: Children with asthmatic attacks present a regular quality of life with
a greater tendency in the female sex and in urban areas. The children indicated
having some level of affectation in the domains of quality of life, being the
limitation of activities and emotions the most affected.

Care practices based on beliefs and customs about the reproductive process have been
present throughout the world since ancient times, and vary depending on the culture of the
region, especially food culture (1). For this reason, the relevance of the study of local cultural
associations stands out, which allows us to get closer to the knowledge of practices (2). Aim:
Describe the cultural dietary practices of pregnant and postpartum women in the Cojitambo
parish, in the context of family structure, 2023. Methodology: Qualitative, phenomenological
study, applied to 15 pregnant and postpartum women from the Cojitambo parish through
convenience sampling. Categories: Family Structure, Food practices, Complicit nature,
Benefits of ancestral food and Cultural transmission. A semi-structured interview and a
genogram were applied to each participant, processing the information with the Wordle
program. The information was triangulated with community leaders and finally the narrative
analysis was carried out. Results: Nutritional practices in the pregnant woman are based on
the benefit of the child, and are developed through organic nutrition; However, in the
postpartum period it has a more cultural connotation, where the mother has a greater role. All
the interviewees complied with the eating practices, evident as repetitive patterns and
consolidated social representations. Conclusion: It is possible to describe the cultural eating
practices of pregnant and postpartum women in the Cojitambo parish. Its generational
transmitters are female figures. They were fulfilled in all families, regardless of their typology.

The deficient system of gray and black water disposal is associated to contagious deceases in human being, animals and plants.
Objective
Determine domestic wastewater management systems in people older than 17 living in San Pablo de Shaglli parish, canton Santa Isabel, during 2014.
Methodology
A descriptive study was done in a 115 person population. Data was obtained by a survey, and was analyzed with SPSS software version 15. Age fluctuated between ages 18-86, and median was 45. 51.3% were men. 63.5% went to elementary school, 3.5 were illiterate, and 6.1% went to college. 52.2% dispose gray water from cooking outdoors; 55.7% of the personal cleaning water also is eliminated outdoors, just as the 6.26% of the laundry grey water, and the 47% dispose black water in septic pit.
Conclusion
More than 50% eliminate grey water from kitchen, personal cleaning and laundry outdoors, and a similar percentage eliminates black water in a septic pit.

Background: Suicide is the third leading cause of death in young people between 15 and 19 years of age; and has been increasing in recent years, which is why it is considered a public health problem.
General objective: To determine the prevalence, characterization and methods of suicide registered in the General Prosecutor's Office of the State of Cuenca, years 2017-2021.
Method: It is a quantitative, observational, descriptive, cross-sectional and retrospective study. It includes all the medical-legal reports of suicides registered in the Prosecutor's Office, during the period January 2017 to December 2021. The information was recorded in a data collection form prepared by the author. The analysis was performed using the statistical program SPSS version 15, where the qualitative and quantitative variables were recorded in frequencies and percentages and their graphic representation. This investigation was approved by the Bioethics Commission of the University, and had the authorization of the Prosecutor's Office.
Results: The prevalence in 2017 was 6.9%, in 2018 it was 6.5%, subsequently increasing to 8.3% in 2020 and 9.7% in 2021. Men represent 73% of the suicide cases. The ages between 20-39 years constitute 49% of the cases. Single people were 62%. People with an education level up to high school were 51%. The most used method of suicide is hanging (81.5%). Suffering from some psychiatric illness is the most frequent pathological antecedent.
Conclusions: The prevalence of suicide in Cuenca increased during the pandemic. Men, single and high school students are the characteristics with the highest frequency of suicide. The most used method of choice is hanging.

Background: In Ecuador for the year 2018, according to the National Transit Agency (ANT), a
total of 1,528 traffic accidents were reported in the province of Azuay, of which 1,357 presented
injuries and 102 died in situ.
Objective: to determine the prevalence and characterization of deaths due to fatal injuries in
vehicular traffic accidents, basin 2015-2018.
Methodology: cross-sectional descriptive study, carried out at the Center for Legal and Forensic
Medicine Cuenca Zone 6, in the period 2015-2018 with 291 records. For the collection of
information, a self-prepared data form was used, the statistics used were the frequency and
percentage.
Results: The prevalence of deaths was 12.1 / 100 thousand inhabitants, the majority of deceased
corresponds to the male sex, with marital status was single, of primary education and with ages
between 19 to 35 years. Running over and collision were the most frequent traffic accident
mechanisms. Injuries considered "Critical" were the most frequent, with a severity index of 61 to 75
points. Cranioencephalic trauma together with cerebral hemorrhage, cerebral edema, were the most
frequent cause of death.
Conclusion: the characteristics of death due to vehicular traffic accidents are similar to those of
the global and regional population, and deaths are lower than the national average.

Introduction: Multiple studies support the association between type 2 diabetes
mellitus (DM2) and the severe clinical course of coronavirus disease 2019
(COVID-19). This systematic review was carried out with the aim of determining
the clinical-epidemiological link between DM2 and COVID-19. Methods: A
systematic review of articles included in the Embase, Web of Science, Cochrane,
PubMed, Scielo and Scopus databases (Q1-Q4), published from February 7,
2020 to October 2021, was carried out. To evaluate the study quality, quality
assessment tools were used. Results: A total of 32 studies were analyzed,
65.6% were single-center, 87.5% were retrospective studies, in 18.8% studies
only patients with COVID-19 and DM2 were included. In 84.3% of the studies,
the predominant sex of the patients was male. In 47% of the studies, the age of
the patients was greater than or equal to 60 years. In addition, 90.6% of studies
reported comorbidities such as hypertension. 87.5% were good quality studies.
Conclusions: The following stand out as points of convergence in the current
scenario of the syndemic between COVID-19 and DM2: advanced age,
deterioration of the immune response, higher levels of inflammatory markers and
the association with complications and comorbidities of DM2. DM2 is considered
a high risk factor for critical clinical conditions and deaths from COVID-19.
Likewise, possible multifaceted pathological mechanistic pathways triggered by
hyperglycemia that cause considerable vulnerability to SARS-CoV-2 infection
have been detailed.

Background: traffic accidents had been increased worldwide, being the bikers,
motorcyclists, and pedestrians the more affected, because of the direct body exposure.
Objective: to determinate the prevalence and characterization of the traffic accidents in
motorcycles. Cuenca, 2018-2020 period.
Methodology: observational, descriptive and transversal study. The data included all the
traffic accidents expedient registered on the EMOV EP system (Empresa Pública Municipal
de Movilidad, Tránsito y Transporte) from Cuenca. Sociodemographic variables and study
variables (individual, environmental and road variables) were collected. The data were
processed in the SPSS program evaluation version, the data are presented in tables, using
frequencies, percentages, standard deviation and arithmetic average.
Results: motorcycle accidents occurred more frequently in men (96%) between 20-39 years
of age (82.7%), with the most frequent type of accident being a crash (68%); these accidents
predominated on weekdays (67.4%), in a good road condition 94.8%. Of the motorcycle
drivers, 86.7% were only injured and only 16.7% were drinking alcohol at the time of the
accident.
Conclusions: the most number of motorcycle accidents happened in men between 20-39
years old, between Monday to Friday working hours, with a minimum of deads; this accidents
were not related with road conditions nor weather conditions.

Background: the state of pregnancy leads women to have some psychological, social and biological
changes; so sexual function can be affected at this stage.
Objective: to identify the prevalence and factors associated with sexual activity in pregnant patients
treated at the Municipal Hospital of Cuenca during the period March - August 2022.
Methodological design: quantitative, analytical, cross-sectional study. The sample consisted of 175
pregnant women from the gynecology area of the Municipal Hospital of Cuenca. A form based on
the EVAS-M questionnaire was applied. The analysis was performed in the statistical programs SPSS
24 and Epi-Info 07.
Results: the average age was 28.05±6.18 years (95% CI: 27.12-28.97), the most frequent age group
was 26-32 years (41.1%); 52.6% were married, 80.6% had a high school education, 64.0% lived in
urban areas, and 84.6% were Catholic. 61.7% had 27 or more gestational weeks; 40% have had a
single gesta; 77.1% have information about sexuality during pregnancy; 42.9% had sexual intercourse
2 times a week; 37.1% had their last sexual intercourse during the first trimester; 63.4% considered
that their self-esteem is medium level and 62.9% that sexual activity causes little harm to the fetus.
Significant mean differences were found for groups defined by age, residence, marital status, level of
education, religion, having information on sexuality during pregnancy, coital frequency, last sexual
intercourse according to trimester of pregnancy, level of self-esteem and considering damage to the
fetus.
Conclusions: sexual dysfunction in pregnant women increases in the third trimester of pregnancy

Target: to determine the predominance and the factors of risk associated with not intentional injuries in girls / you 5-year-old minors, urban parish of Mercury 2016.
Materials and methods: quantitative cross section investigation, with a sample of 108 children´s; there was used the observation and the application of survey. The information was gathered and stored in the database in the program SPSS 15.00. The descriptive statistics was used and for the association between factors of risk and not intentional injuries the RP with an IC 95 % and the value of p <0.05.
Results: the life expectancy of the infantile population was 2.7 where 55 % corresponds to the masculine sex. Not intentional injuries caused in the group from 1.01 to 2 years in 85.7 %% (p <0.001), having easy access to the kitchen in 86.7 % (RP 1.3; IC 95 %: 0.8 – 2) and belonging to extensive families in 73.3 % (p=0.02; IC 95 %: 1 – 1.99). The predominance of not intentional injuries was 71.7 %, the main cause the falls 81.3 %, generating in most of cases TEC 53.4 %, the same that 63.9 % was attended in the hearth.
Conclusions: the predominance of not intentional injuries in children´s younger than 5 years is high and it collaborates with the age of the child, the belonging to extensive families and the access to areas like the kitchen.

The SARS-CoV-2 virus has challenged the health systems of the Latin American continent causing a health, economic and social phenomenon. The efficiency with which it spreads throughout Latin America, infecting the population, suggests that it is much more difficult to contain. This situation pushes to the adoption of radical measures that generate a massive population and epidemiological impact such as confinement, a social, economic and sanitary measure that is conceptualized as the most radical strategy to slow down the massive contagion and flatten the epidemic curve, but this measure has great implications in the reality of the population.

Compulsive exercise is considered a sustained rigid repetitive behavior that represents a morbid behavioral pattern. Indeed, it has been associated with eating disorders that can trigger pathologies such as anorexia nervosa, bulimia nervosa and other unspecified eating disorders. Objective: To determine the prevalence of compulsive exercise and its association with risk behaviors for eating disorders in a population aged 20 to 34 years in gyms de Cuenca, 2021. Methodology: Observational, quantitative, cross-sectional prevalence study in 425 users of gyms in Cuenca, using two questionnaires: Compulsive Exercise Test (CET) and Eating Disorders Examination (EDE-Q). Data processing was carried out with SPSS version 23 and Epidat 3.1 and an analysis of frequencies and percentages with PR and 95% CI was elaborated. Results: 78.1% were aged between 20 and 27 years. Female gender 56 %, urban residence 62.2 %, single marital status 77 % and third level academic degree with 64 % predominated. A prevalence of compulsive exercise of 16 % and of eating disorder behaviors of 10.8 % was observed. Statistical significance was evidenced between eating disorder risk behaviors and the variable of compulsive exercise with a p-value < 0.05. Conclusion: There was a statistically significant relationship between compulsive exercise and eating disorders among the users who attended gyms in the city of Cuenca.

Background: Although violent deaths in Ecuador remain low compared to other countries in the region, they have shown an increase in recent years. With the arrival of the SARS-CoV-2 pandemic, is important to measure the real statistical and epidemiological data of violent deaths, which were documented during the confinement period of the 2020 National State of Exception
Objective: To determine the epidemiology and medical-legal profile of violent deaths in Ecuador during the State of Exception, year 2020
Methodology: Descriptive, observational, cross-sectional, and retrospective study. A data collection form was made and used as in instrument in this study. The data was tabulated in Microsoft Excel, calculating frequency, percentage, and measures of dispersion.
Results: Violent deaths were more prevalent than previous years in young adult males of Ecuadorian nationality in urban areas. Violent deaths varied by geographical sub-area, as well as by cause. Firearms were found to be more commonly used in homicides, while constrictor weapons were more commonly documented in suicides. Both homicides and suicides were more common during weeks when fines were in place to maintain home confinement. However, accidental deaths, such as those caused by falls and precipitations were more commonly documented on Fridays.
Conclusions: The results obtained do not differ significantly from those obtained in previous national or international studies. Violent deaths continue to be a public health problem. Further study is needed to determine the impact that the COVID-19 pandemic had on the rates and types of violent death in Ecuador

Background: Malnutrition in critically ill patients has gained importance; studies have shown that obesity and malnutrition are independent risk factors for mortality, leading to a longer stay in intensive care and time on mechanical ventilation. In our country there is no data on malnutrition in critically ill patients or any relationship with mortality. Objective: Determine mortality and its relationship with nutritional status in patients in the Intensive Care Unit of the Vicente Corral Moscoso Hospital from October 2021 to January 2022. Methodology: Analytical, prospective observational study, clinical records were reviewed collecting sociodemographic data, biochemical and anthropometric parameters, arm and calf circumference. Measures of central tendency and dispersion were calculated, Chi2 to measure association, relative risk, 95% confidence interval and p value (< 0.05) for statistical significance. Results: 81 patients, 46% aged between 20 and 44 years, 67% male, 85.2% with enteral nutrition,21% died. 43% of adults were overweight, 52% of older adults had a normal BMI, 46.9% had a moderate risk of malnutrition according to CONUT. Albumin values were lower at discharge, in deceased patients (2.39 mg/dl) and in living patients (2.83 mg/dl) (P= 0.019). Those malnourished according to calf circumference had higher mortality (P=0.030) with a RR 3.68 95% CI (1.08-12.5). Conclusions: There is a high percentage of overweight with a higher risk of death, there was no relationship between anthropometric or biochemical variables at admission, at discharge there was a relationship between mortality with albumin levels and malnutrition according to calf circumference.

Background: It has been proven in several studies that the use of drugs able to prolong the QT interval corrected by Bazzet’s equation (QTcB) increases with age and cardiac diseases; therefore, an electrocardiogram screening may provide valuable information about cardiovascular vulnerability of the elderly.
Goal: To identify the prolongation’s prevalence of the interval QTcB and the clinical-pharmacological factors associated among the elderly staying in gerontological and geriatric centers in the urban area of Cuenca.
Methods: A cross-sectional study involving 363 elderly residents of gerontological and geriatric centers of the urban area of Cuenca, which had an agreement with the Ministry of Economic and Social Inclusion during the year 2016; besides, all participants signed the informed consent. Participants were selected from seven different centers in a weighted and random fashion. A previously validated questionnaire was used; and, a EKG using standardized techniques was obtained. The QTc interval was defined using the Bazett’s equation; prevalence ratio with 95% CI and p<0.05 of significance was used to define the association analysis. Binary logistic regression was used for the bivariate analysis. The statistical software packages used were SPSS v19 and Epi Info v7.
Results: The average age of the participants was 80±10 years, 52% were male and 48% female; 49,3% were singles and 25,6% were widowed; 90,4% were economically inactives, 78% had primary education. The prevalence of QTcB prolongation was 17,9%. Arterial hypertension, diabetes type 2, atrial fibrillation, cardiac failure, heart attack, bradycardia and hypotiroidis showed don’t be an associated factor. Significant association between chronic renal failure, valproic acid, allopurinol, alprazolam, clonazepam, and fluoxetine was found.
Conclusions: The prolongation of the QTcB interval and the use of drugs with potential risk to prolong the aforementioned interval in elderly people was high
