Abstract:
Background: Acute cholecystitis is the leading cause of hospital morbidity in women and the fifth in men in Ecuador.
Objective: To apply the Tokyo Guidelines 2013 in the diagnosis of acute cholecystitis at Hospital José Carrasco Arteaga 2014-2016.
Methodology: A descriptive and retrospective research was performed. The universe consisted of 182 patients with anatomopathological diagnosis of acute cholecystitis. The information was collected in forms, the data were processed using the PSPP V10 program by absolute frequency and percentage, which were represented in tables.
Results: 39% of the patients were 40 to 64 years old and 54.9% were men. All showed a local sign of inflammation, 78% represented systemic signs of inflammation and 91.8% had pathological findings. 72% of the patients with pathological diagnosis of acute cholecystitis were also diagnosed as definitive diagnosis of the disease according to Tokyo Guidelines 2013. 59.9% presented mild cholecystitis and only 6% severe.
Conclusions: The most affected patients were male adults, in most cases the pathological diagnosis of acute cholecystitis coincided with the definitive diagnosis of the disease according to the criteria of Tokyo Guidelines 2013. It is necessary to establish a protocol for the use of Tokyo Guidelines 2013 in the routine management of patients with suspected acute cholecystitis

Antecedentes: La colecistitis aguda ocupa el primer lugar como causa de morbilidad a nivel hospitalario en mujeres y la quinta en hombres en el Ecuador.
Objetivo: Aplicar la Guía de Tokio 2013 en el diagnóstico de colecistitis aguda, Hospital José Carrasco Arteaga 2014-2016.
Metodología: Se realizó una investigación descriptiva y retrospectiva. El universo estuvo conformado por 182 pacientes con diagnóstico anatomopatológico de colecistitis aguda. La información fue recolectada en formularios, se procesaron los datos utilizando el programa PSPP V10 mediante frecuencia absoluta y porcentaje, que fueron representados en tablas.
Resultados: El 39% de los pacientes tienen de 40 a 64 años y el 54,9% eran hombres. Todos manifestaron algún signo local de inflamación, el 78% presentó signos sistémicos de inflamación y el 91,8% tuvo hallazgos de imagen patológicos. El 72% de los pacientes con diagnóstico anatomopatológico de colecistitis aguda también tuvieron diagnóstico definitivo de la enfermedad según la Guía de Tokio 2013. El 59,9% presentó colecistitis leve y solo el 6% grave.
Conclusiones: Los pacientes más afectados fueron los adultos varones, en la mayoría de casos el diagnóstico anatomopatológico de colecistitis aguda coincidió con el diagnóstico definitivo de la enfermedad según los criterios de la Guía de Tokio 2013. Es necesario protocolizar el uso de la guía en el manejo habitual de los pacientes con sospecha de colecistitis aguda.
