| Abstract: | Background. In regional anesthesia epidural space identification is a procedure that requires refined skills. Some devices have attempted to facilitate the approach, but the main difficulty has been the widespread use. Recently it has been reported the usefulness of a stopcock which placed in the drum of the Tuohy needle becomes a variation of the pendant drop.
Objective. To determine the usefulness of a variant of the technique of hanging drop Gutierrez to identify the epidural space.
Material and methods. With simple random sampling 155 patients scheduled for lower body surgery under epidural anesthesia were included . A variant of the hanging drop was used Gutierrez through a stopcock attached to the drum of the Tuohy needle 18G to identify the epidural space. Was considered as a sign of correct identification of the epidural space to absorption of anesthetic vertical branch of the device and the percentage of success was measured.
Results. The age of the sample was 33.9 ± 14.6 years old (18-75 years). The weight was 33.9 ± 14.6 kg (50-98 kg) and height 1.59 ± 0.6 m. Normal weight was found in 40%, overweight in 39.4% and obesity in 20.6 %.
Space was located correctly in 98.1% of cases with an error of 1.9%. Compared to 18% error reported with classical technique Gutiérrez the difference was highly significant (P = 0.0004)
Discussion. The correct localization of the epidural space with a simple and easy device management is emerging as getting a fully valid alternative especially for anesthesiologist in training.

| Resumen : | Antecedente. En la anestesia regional la identificación del espacio epidural es un procedimiento que requiere destreza refinada. Algunos dispositivos han intentado facilitar el abordaje, pero la mayor dificultad ha sido la generalización de su uso. Recientemente se ha notificado la utilidad de una llave de tres vías que colocada en el tambor de la aguja de Tuohy se convierte en una variante de la gota pendiente.
Objetivo. Determinar la utilidad de una variante de la técnica de la gota pendiente de Gutiérrez para identificar el espacio epidural.
Material y métodos. Con un muestreo aleatorio simple se incluyeron155 pacientes programados para cirugía de hemicuerpo inferior bajo anestesia epidural. Se utilizó una variante de la gota pendiente de Gutiérrez mediante una llave de tres vías acoplada al tambor de la aguja de Tuohy de 18G para identificar el espacio epidural. Se consideró identificación correcta del espacio epidural a la absorción del anestésico de la rama vertical del dispositivo y se midió el porcentaje de éxitos.
Resultados. La edad de la muestra fue de 33,9 ± 14,6 años (18 - 75 años). El peso fue de 68,3 ± 9,04 kg (50 -98 kg) y la estatura de 1,59 ± 0,6 m. El peso normal se encontró en el 40%, el sobrepeso en el 39,4% y la obesidad en el 20,6%.
Se localizó correctamente el espacio en el 98,1% de los casos con un error del 1,9%.Comparando con el 18% de error reportado con la técnica clásica de Gutiérrez, la diferencia fue altamente significativa (P = 0,0004)
Discusión. La localización correcta del espacio epidural con un dispositivo de manejo sencillo y fácil de conseguirse perfila como una alternativa plenamente válida sobre todo para el anestesiólogo en formación.
