dc.description | Antecedentes: El cáncer de mama es una de las patologías más frecuentes en mujeres, con más de 2.3 millones de casos en 2020 y una tasa de incidencia de 132 casos por cada 100000 habitantes. Es la segunda neoplasia maligna más frecuente en la población femenina de Cuenca, con la más alta tasa de incidencia en el período 2010-2014, según las estadísticas de SOLCA, Cuenca. Se ha comprobado que el CA 15.3 y el CEA contribuyen de manera importante en el control, seguimiento y pronóstico de la enfermedad. Objetivo: Analizar y comparar los marcadores CA 15.3 y CEA para evaluar la respuesta al tratamiento y seguimiento del cáncer de mama de pacientes que acuden al Hospital de Especialidades José Carrasco Arteaga en el período 2016-2019. Metodología: La presente investigación es de evaluación de la eficacia de una prueba clínica en el control y seguimiento de pacientes con cáncer de mama. Se recopilaron los datos de las historias clínicas de las pacientes según los criterios de inclusión, por el sistema MIS AS 400, tabulando en Excel. Posteriormente se analizaron los datos mediante estadística descriptiva e inferencial utilizando R Studio, IBM SPSS V22 y Microsoft Excel. Resultados: El tipo de cáncer más frecuente es el carcinoma ductal invasivo (62.58%) y el estadio más común es el IIB (23.13%). El CA 15.3 posee una SE: (0.81), SP: (0.93), VPP: (0.81) y VPN: (0.93), mientras que el CEA posee una SE: (0.75), SP: (0.87), VPP: (0.60) y VPN: (0.93), CA15.3-CEA poseen una SE: (0.80), SP: (0.97), VPP: (0.80) y VPN: (0.97). Los puntos de corte fueron; CA 15.3 (29.41 U/mL), CEA (2.71 ng/mL) y conjuntamente (19.84 U/mL y 2.58 ng/mL). Se estableció que existe relación entre la concentración del marcador tumoral y la presencia de metástasis (p<0.05). Igualmente se confirmó la existencia de relación entre el estadio y la tasa supervivencia (p<0.05) y se evidenció una relación entre el incremento de la concentración y el estadio. Conclusiones: Se determinó que el CA 15.3 posee mayor SE, SP, VPP y VPN que el CEA. Finalmente, la asociación CA15.3-CEA mejora los parámetros y justifica su empleo en la práctica clínica para evaluar el tratamiento, seguimiento y pronóstico del paciente. | en_US |

It is the second most frequent malignant neoplasm in the female population of Cuenca, with the highest incidence rate in the period 2010-2014, according to statistics from SOLCA, Cuenca. CA 15.3 and CEA have been found to contribute significantly in the control, follow-up and prognosis of the disease. Objective: To analyze and compare CA 15.3 and CEA markers to evaluate the response to treatment and follow-up of breast cancer in patients attending the Hospital de Especialidades José Carrasco Arteaga in the period 2016-2019. CA 15.3 has an SE: (0.81), SP: (0.93), PPV: (0.81) and NPV: (0.93), while CEA has an SE: (0.75), SP: (0.87), PPV: (0.60) and NPV: (0.93), CA15.3-CEA has an SE: (0.80), SP: (0.97), PPV: (0.80) and NPV: (0.97). The cut-off points were: CA 15.3 (29.41 U/mL), CEA (2.71 ng/mL) and jointly (19.84 U/mL and 2.58 ng/mL). It was established that there is a relationship between the concentration of the tumor marker and the presence of metastasis (p<0.05). Likewise, the existence of a relationship between stage and survival rate was confirmed (p<0.05) and a relationship between the increase in concentration and stage was evidenced. Conclusions: It was determined that CA 15.3 has higher SE, SP, PPV and NPV than CEA. Finally, the association CA15.3-CEA improves the parameters and justifies its use in clinical practice to evaluate the treatment, follow-up and prognosis of the patient. | en_US |
