| Abstract: | Objective: To determine the validity of the concentration of creatinine in the vaginal fluid, to diagnose premature rupture of membranes (PROM); using the clinical diagnosis as a reference method, in pregnant women between 24-42 weeks of gestation who attended the Vicente Corral Moscoso Hospital, during 2017-2018.
Method and materials: A diagnostic test validation study was conducted. The sample size was based on the prevalence of PROM (15%), confidence level 95%, error inference 4%; and infinite population. The data was collected directly and analyzed with the SPSS software. The PROM was diagnosed clinically, vaginal wash was taken from pregnant women with rupture and without rupture of membranes; the validity of the test was determined by sensitivity, specificity, positive and negative predictive value.
Results: The minimum age was 14 years, the maximum age 44 and the median age 25; the mean gestational age of 37.4 weeks of gestation. About the hours of PROM, 1 hour was the minimum, and 24 hours the maximum. The concentration of creatinine in the vaginal discharge of pregnant women with PROM ranged between 0.45 mg / dl and 3.35, median of 0.97; in pregnant women without rupture between 0 and 0.65, median of 0.1. The prevalence was 10.9%; sensitivity 96.7%, specificity 96.3%, positive predictive value 76.3%, negative predictive value 99.6%, negative likelihood ratio 0.03 and positive likelihood ratio 26.13.
Conclusion: The determination of creatinine in vaginal fluid is a valid test to diagnose PROM. |
| Description: | Objetivo: Determinar la validez de la concentración de creatinina en el fluido vaginal, para diagnosticar ruptura prematura de membranas (RPM); utilizando el diagnóstico clínico como método de referencia, en embarazadas entre 24-42 semanas de gestación que acudieron al Hospital Vicente Corral Moscoso, durante el 2017- 2018.
Método y materiales: Se realizó un estudio de validación de prueba diagnóstica. El tamaño de la muestra se efectuó sobre la base de la prevalencia de RPM (15%), nivel de confianza 95%, error inferencia 4%; y población infinita. Los datos se recolectaron de manera directa y se analizó con el software SPSS. La RPM se diagnosticó clínicamente, se tomó muestra del lavado vaginal a las embarazadas con ruptura y sin ruptura de membranas; la validez de la prueba se determinó por la sensibilidad, especificidad, valor predictivo positivo y negativo.
Resultados: La edad mínima fue de 14 años, la máxima 44 y la mediana 25; la edad gestacional media de 37,4 semanas de gestación. Sobre las horas de RPM, 1 hora fue lo mínimo, y 24 horas lo máximo. La concentración de creatinina en el flujo vaginal de gestantes con RPM fluctuó entre 0,45 mg /dl y 3,35, mediana de 0,97; en gestantes sin ruptura entre 0 y 0,65, mediana de 0,1. La prevalencia fue 10,9%; la sensibilidad 96,7%, especificidad 96,3%, valor predictivo positivo 76,3%, valor predictivo negativo 99,6%, razón de verosimilitud negativa 0,03 y razón de verosimilitud positiva 26,13.
Conclusión: La determinación de creatinina en fluido vaginal, es una prueba válida para diagnosticar RPM. |
